With the Level II Cath Lab now at Upper Valley, patients experiencing heart attacks can receive life-saving care closer to home. In addition, non-emergency heart blockages can also be diagnosed and treated at Upper Valley. With the added availability and capabilities of experienced interventional cardiologists, patients who are experiencing heart problems can now be diagnosed and treated at Upper Valley. Form 990, Part III, line 4c (continued): The UVMCF underwrote the cost of Rachel's Challenge for students in Miami County. In 2018, seven schools and 8,822 students took part in Rachel's Challenge programming. Form 990, Part III, Line 4d: This number represents the balance of total expenditures of temporarily restricted funds for Upper Valley Medical Center net of programs described in service accomplishments. Form 990, Part VI, Section A, Line 4: THE CODE OF REGULATIONS OF UVMCF UPDATED JULY 30, 2018 STATES THAT UVMCF SHALL HAVE NO MEMBERS. Form 990, Part VI, Section B, Line 11B: This 990 filing and attached schedules (the return) are prepared by a staff member in the Tax Department of Premier Health (Premier), with which the UVMCF is financially interrelated. The return is reviewed by the Tax Manager and the Director of Tax Compliance of Premier. The return is concurrently sent to Ernst & Young U.S. LLP for their review. After all changes from the above groups are made, the return is reviewed by the Vice President/Controller of Premier and the Chief Financial Officer of this entity. A final version of the return is sent to Ernst & Young U.S. LLP for a final review and then provided to the Board of Trustees for review. At the Board of Trustees' meeting, the Vice President/Controller of Premier (or designee) shares detailed information on compensation and other key areas, as well as address any other questions from the Board of Trustees, pending their review. The 990 filing is also shared with the Audit Committee of Premier. The UVMC Foundation (UVMCF) makes every effort to always follow the intent and wishes of all its donors. The intended uses of the organization's endowment funds and/or earnings from endowment funds are driven by the wishes/restrictions placed on the funds by the donor. A Permanent endowment results when a donation is received with a restriction that the principal or gift amount is to be retained in perpetuity and cannot be spent. The earnings from the permanent endowment are used for the purpose specified by the donor. Temporary endowments are funds in which all or part of the principal may be expended only after the expiration of a stated period of time or occurrence of a specified event, depending on the donor's wishes. Earnings from temporarily restricted endowments can be either unrestricted or restricted. The UVMCF uses the temporarily restricted endowments for the intended purpose set up by the donor, only after the period of time or specified event set by the donor has been met. The categories of restricted endowments are as follows: Education (17%); provide for Long-term Care and Rehabilitation (17%); Cancer Care/Center (10%); Emergency and Trauma Care (1%); Other Hospital Programs (15%) and miscellaneous (40%). With the help of the UVMC Foundation (UVMCF) in purchasing life-saving equipment, the Level II Cath Lab saw 165 patients within a six-month period and has saved countless lives. Prior to the opening of the Level II Cath Lab at Upper Valley, patients suffering from heart attacks would have been emergently transferred to Miami Valley Hospital delaying life-saving care.
