The form 990 is prepared by an independent accounting firm. It is then reviewed by the cfo, comparing it to the audited financials. Following approval by the cfo, it is then provided to the ceo/president for his review, and then to the board for approval. Once accepted, it is filed with the irs and then made available to all governing members.

All board members are required to sign an annual conflict of interest statement disclosing any potential conflicts of interest.

Financial Statement Notes

PART X, LINE 2:

The organization is a nonprofit, publicly supported organization, as defined in section 501(c)(3) of the internal revenue code ("code") that is exempt from federal income taxes under section 501(a) of the code. Accordingly, no federal income taxes are recorded in the accompanying financial statement. Unrelated business income, if any, is insignificant and no tax provision has been made in the accompanying financial statements. The organization following financial accounting standards board accounting standards of codification ("fasb asc") no. 740-10, relating to accounting for uncertain tax positions. Asc 740-10 prescribes a recognition threshold and measurement process for accounting for uncertain tax positions and also provides guidance on various related matters such as derecognition, interest and penalties and disclosure requirements. The organization does not have any entity level uncertain tax positions in connection with these financial statements. The organization files a us federal tax return.

WITH ONLY ONE LIMITED-CAPACITY HOMELESS SHELTER IN DENTON COUNTY AND LITTLE TO NO AFFORDABLE HOUSING IN THE AREA, CCA IS A VITAL RESOURCE FOR FAMILIES AT RISK OF HOMELESSNESS DUE TO EVICTION OR FORECLOSURE. AS A RESULT OF CCA'S HOMELESS PREVENTION PROGRAM AND CASE MANAGEMENT, 2240 INDIVIDUALS IN THE PREVIOUS YEAR. ACCORDING TO THE UNITED STATES INTER-AGENCY COUNCIL OF HOMELESSNESS REPORTS THAT HOMELESS INDIVIDUALS CAN COST TAXPAYERS AS MUCH AS $30,000-$50,000 ANNUALLY. CCA HAS POSSIBLY SAVED NORTH TEXAS TAXPAYERS $67,200,000 LAST YEAR.

APPROXIMATELY 75% OF OUR PATIENTS ARE CHRONICALLY ILL AND SUFFER ILLNESSES SUCH AS HEART DISEASE, STROKE, DIABETES, CHRONIC LOWER RESPIRATORY DISEASE, AND CIRRHOSIS. WHEN A PATIENT COMES TO THE AHC, WE BECOME THEIR PRIMARY MEDICAL HOME UNTIL THEY ARE ABLE TO SECURE FINANCIAL STABILITY AND HEALTH INSURANCE THAT MEETS THEIR SPECIFIC NEEDS. THE AHC ACTS AS FIRST-IN-LINE CARE FOR EVERY PATIENT, WITH SERVICES THAT IMPROVE THEIR HEALTH AND QUALITY OF LIFE. AS THEIR MEDICAL HOME, PATIENTS WILL ACCESS CLINIC SERVICES AN AVERAGE OF THREE TO FIVE TIMES ANNUALLY. WITH THESE PEOPLE ACCESSING OUR MEDICAL SERVICES, WE AVERT AN ESTIMATED $1.6 MILLION IN UNPAID HOSPITAL EMERGENCY ROOM BILLS.

TODAY, CCA OPERATES ONE OF THE LARGEST CLIENT-CHOICE FOOD PANTRY IN NORTH TEXAS, PROVIDING FOOD, PERSONAL HYGIENE AND HOUSEHOLD PRODUCTS TO MORE THAN 500 FAMILIES EVERY WEEK. LAST YEAR CCA'S FOOD PANTRY DISTRIBUTED MORE THAN 2,000,000 POUNDS OF FOOD, INCLUDING 850,000 POUNDS OF FRESH FRUITS AND VEGETABLE TO 1,938 FAMILIES.

CCA PARTNERS WITH VARIOUS COMMUNITY GROUPS FOR A BACK TO SCHOOL PROGRAM TO PROVIDE TWO NEW OUTFITS OF CLOTHING, A NEW PAIR OF SHOES, SOCKS, UNDERWEAR, SCHOOL SUPPLIES AND NEW BACKPACKS TO CHILDREN WHO MIGHT OTHERWISE START THE SCHOOL YEAR WITHOUT THESE BASIC NECESSITIES. THE CHRISTMAS PROGRAM ALLOWS CLIENTS TO SHOP FOR GIFTS FOR THEIR CHILDREN DESPITE THE HARDSHIPS THEY CURRENTLY FACE. CCA ALSO PROVIDES SPIRITUAL SERVICES AND PROGRAMS DIRECTED TOWARD THE SPIRITUAL AND EMOTIONAL WELLNESS OF OUR CLIENTS, OFFERING HOPE AND DIRECTION THROUGH CHURCH REFERRALS, PASTORAL PRESENCE, AND SPIRITUAL COUNSELING.
