IRS990/Description0Heights and Hills promotes successful aging in the community. Since 1971, Heights and Hills has helped people live their older years on their terms, independently and with dignity. Today, Heights and Hills is one of the largest providers of social services for older adults in Brooklyn annually serving over 3,000 older adults and the families and friends who care for them throughout 19 neighborhoods, through the provision of case management, caregiver services and transportation programs.As a supplement to the compassionate and professional work done by a multi-lingual staff ensuring the physical and emotional supports of homebound older adults, our Bridge-to-Home program that began in January 2013 serves the frailest of seniors who face multiple barriers to independent living post hospitalization. Other Heights and Hills services offered include volunteer recruitment and support through medical escorting, telephone reassurance and friendly visiting.During the 2013 fiscal year:- 1,113 Brooklyn seniors and their family members and friends were provided information and referrals on issues as wide-ranging as eligibility for entitlements and benefits, services available through Medicare, how to get access to wheelchairs, housing options available to seniors, and much more.- Our case managers logged over 14,000 hours while providing 1,278 homebound older adults with in-home assessments and on-going follow-up.- We served over 500 individual caregivers with much-needed information, counseling and support. Our caregiver program staff ran five separate support groups: - A support group for caregivers at the Heights and Hills office - A support group at Mark Morris Dance Group for caregivers of older adults with Parkinson's - A support group for caregivers at the Remsen Senior Center in Crown Heights - An evening telephone support group for caregivers - A bereavement support group for caregivers whose loved one has passedWe also provided over 2,800 hours of respite homecare to the friends, family and loved ones caring for an older adult in our community.- Heights and Hills staff made calls to hundreds of isolated and at-risk individuals to make sure they were prepared and remained safe during severe weather, such as heat and snow advisories.- We educated the public through presentations at meetings, on panels, and by joining in several collaborations, including Brooklyn Local Aging Networks Alzheimers Conference, DOROT, Emmanuel Baptist Churchs Prime Time Ministry, Geriatric Mental Health Alliance, Long Island University International Drug Information Center, Red Hook Initiative, and numerous local health fairs.- As a lead partner with New York Methodist Hospital, and in collaboration with five skilled nursing facilities, two home care agencies and a physician home visiting practice our community care transitions program, Bridge to Home, began in January 2013 with funding provided by the Center for Medicare and Medicaid Services. Aimed at reducing hospital readmissions, Bridge to Home has seen tremendous success in its first year, serving over 300 participants and feeding numerous clients and their families through to additional agency supports, such as caregiver services and volunteer assistance.- At the end of 2013 our Volunteer Program recruited 40 new volunteers with elderly, homebound case management clients, providing over 1,400 hours of friendly visiting, medical escorting, household assistance and telephone reassurance. Heights and Hills programs and services are poised to expand in 2014, helping us to serve even more older adults in need in Brooklyn. Our mission is critical now more than ever with an area of service that covers one-third of Brooklyn's community districts which spans a diverse ethnic, racial, linguistic, religious and socioeconomic territory, all while forecast data shows older adults turning 65 at a faster rate each year.
