Supplemental Narrative

Additional Explanations

Pt III, Line 2

Play for the A's - a community effort led by a local high school lacrosse program to raise funds and provide support for Alzheimer's patients and their caregivers in their home environments.

Pt VI, Line 11B

Form 990 is drafted by the organization's Controller

Pt VI, Line 11B

and President. It is then saved as an electronic

Pt VI, Line 11B

file and transferred to a visual media presentation.

Pt VI, Line 11B

The draft Form 990 is then electroncially delivered

Pt VI, Line 11B

to the three-member Audit Committee. Questions and

Pt VI, Line 11B

answers are coordinated with President and Controller.

Pt VI, Line 11B

The audit committee approves the filing of the 990,

Pt VI, Line 11B

reports on such approval to the Board, and ensures that

Pt VI, Line 11B

each board member is aware that the 990 is available for

Pt VI, Line 11B

their own personal review.

Pt VI, Line 12C

Each new board member undergoes an initiation process

Pt VI, Line 12C

with the President. As part of that process, the written

Pt VI, Line 12C

conflict of interest policy is reviewed with the new

Pt VI, Line 12C

member, who is then requested to complete the conflict of

Pt VI, Line 12C

interest questionnaire for submission at or prior to

Pt VI, Line 12C

his/her first active board meeting. Active board members

Pt VI, Line 12C

are required to update their personal conflict of

Pt VI, Line 12C

interest questionnaire at an annual board meeting. The

Pt VI, Line 12C

Chairperson of the Board and President review the

Pt VI, Line 12C

completed questionnaires, and the Chair routinely reminds

Pt VI, Line 12C

board members of the need or suggestion to abstain from

Pt VI, Line 12C

related votes, if warranted.

Pt VI, Line 15B

The President of the organization prepares an annual

Pt VI, Line 15B

review of the Taylor Hospice Executive Director, the

Pt VI, Line 15B

Controller, and the Foundation Operations Manager, and

Pt VI, Line 15B

shares the review content with the Compensation

Pt VI, Line 15B

Committee. The Compensation Committee is comprised of

Pt VI, Line 15B

a cross-section of board members, yet operates

Pt VI, Line 15B

independently of the Board under budget constraints that

Pt VI, Line 15B

may be imposed by the Board's Finance Committee. As part

Pt VI, Line 15B

of the review by each individual, the President submits

Pt VI, Line 15B

salary history, inflation statistics, internal budgetary

Pt VI, Line 15B

information, and comparability data to committee members,

Pt VI, Line 15B

and makes recommendations for consideration in the area

Pt VI, Line 15B

of gross compensation and employer-funded retirement and

Pt VI, Line 15B

and health benefit contributions.

Pt VI, Line 15A

The Chairperson of the Board prepares an independent

Pt VI, Line 15A

assessment of the President to the Compensation

Pt VI, Line 15A

Committee members, collects comparability data, and

Pt VI, Line 15A

utilizes some of the data presented by the President

Pt VI, Line 15A

related to other key employees with the objective of

Pt VI, Line 15A

achieving committee agreement on appropriate levels of

Pt VI, Line 15A

compensation for the President.

Pt VI, Line 19

Governing documents, conflict of interest policy, and

Pt VI, Line 19

financial statements are available to the public upon

Pt VI, Line 19

request. Financial statements are also posted to the

Pt VI, Line 19

website, as well as the most recently filed Form 990

Pt VI, Line 19

with certain sensitive information redacted.

Financial Statement Notes

Pt X, Line 2

Taylor Community Foundation is exempt from federal

Pt X, Line 2

income tax under Section 501(c)(3) of the Internal

Pt X, Line 2

Revenue Code. However, income from certain activities

Pt X, Line 2

not directly related to the Foundation's tax exempt

Pt X, Line 2

purpose is subject to taxation as unrelated business

Pt X, Line 2

income. In addition, the Foundation qualifies for the

Pt X, Line 2

charitable contribution deduction under Section 170(b)(1)(A)

Pt X, Line 2

and has been classified as an organization that is not

The Foundation follows professional standards which clarify

Pt X, Line 2

the accounting for uncertainty in income taxes recognized

Pt X, Line 2

in the Foundation's financial statements and prescribe

Pt X, Line 2

a recognition threshhold and measurement attribute for

Pt X, Line 2

the financial statement recognition and measurement of

Pt X, Line 2

a tax position taken or expected to be taken on a tax return.
