To connect volunteeers and donors to support grants, programs, and scholarships that change lives in the Delaware County, Pennsylvania region.

Promote health and wellness Taylor Community Foundation is committed to initiate and support programs and organizations that advance health, wellness, education, and community spirit within the TCF Service Area. Our primary program is the operation of Taylor Hospice.

TAYLOR HOSPICE SERVED A TOTAL OF 153 PATIENTS IN THEIR HOMES IN 2014-2015. EVERY PATIENT'S UNIQUE NEEDS ARE ASSESSED ON AN ONGOING BASIS. NURSES, HOME HEALTH AIDES, AND ALLIED HEALTH PROFESSIONALS PROVIDE FOR THE PATIENT'S CLINICAL AND OTHER PHYSICAL NEEDS. WE ASSIST FAMILIES IN OBTAINING MEDICINES AND MEDICAL EQUIPMENT AND ATTEND TO PATIENT'S GROOMING AND HYGIENE. ALL OF OUR CLINICAL CARE PROVIDERS ARE FULLY LICENSED AND TRAINED IN THE DISCIPLINE OF HOSPICE CARE. PERSONAL CARE PLANS ARE DEVELOPED AND EMERGENCY SERVICES ARE AVAILABLE 24-HOURS-A-DAY, SEVEN-DAYS-A-WEEK. TAYLOR HOSPICE SOCIAL WORKERS ARE AVAILABLE TO HELP INDIVIDUALS COPE WITH THE MANY CHALLENGES THAT ACCOMPANY THE TERMINAL ILLNESS OF A LOVED ONE. WE REMAIN ON HAND TO PROVIDE EMOTIONAL SUPPORT AND TO ASSIST THE FAMILY IN COMPLETING FORMS FOR INSURANCE COMPANIES AND FINANCIAL INSTITUTIONS. IN ADDITION, OUR SOCIAL WORKERS CAN IDENTIFY RESOURCES IN THE COMMUNITY THAT MAY PROVIDE FURTHER ASSISTANCE, IF NEEDED. OUR CHAPLAIN WILL WORK WITH THE FAMILY'S DESIGNATED CLERGY PERSON, IF DESIRED. IN THIS WAY, THE PATIENT AND FAMILY RECEIVE SPIRITUAL SUPPORT CONSISTENT WITH THE TRADITIONS OF THEIR CHOSEN FAITH.
