The Coalitions current Executive Director was given a copy of the completed Form 990 to review prior to its filing. Any questions that arose during the review of the Form 990 were addressed prior to its filing. A copy of the completed Form 990 was also made available to the entire Board of Directors.

Form 990, Part VI, Section B, Line 12C

The Coalition provides a copy of its Conflict of Interest policy to its Board of Directors, officers, and staff. On an annual basis, the Board must sign and complete an acknowledgement form, disclosing any potential conflicts. In addition, disclosure is required throughout the year, whenever a conflict arises with any Board member, officer, or staff person. Disinterested members of the Board make a determination as to whether a conflict does exist and what subsequent action is needed.

Form 990, Part VI, Section B, Line 15

Compensation for the Executive Director and other employees is reviewed annually by the Board of Directors. The Board votes on the compensation that will be given to said employees at an annual meeting where the upcoming budget is reviewed.

The Coalitions Articles of Incorporation have been filed with the Indiana Secretary of State SOS and are available online at the SOSs website for viewing. Governing documents, a conflict of interest policy, and financial statements are available to the public upon request. The Coalition can be reached by the public in several ways, including phone, website, mail, and e-mail.

Disinterested members of the Board make a determination as to whether a conflict does exist and what subsequent action is needed.
