The Coalitions current Executive Director and its President of the Board of Directors were given a copy of the completed Form 990 to review prior to its filing. Any questions that arose during their review of the Form 990 were addressed prior to the filing of the Form 990.

Form 990, Part VI, Section B, Line 12C

The Coalition provides a copy of its Conflict of Interest policy to its Board of Directors, officers, and staff. On an annual basis, the Board must sign and complete an acknowledgement form, disclosing any potential conflicts. In addition, disclosure is required throughout the year whenever a conflict arises with any Board member, officer, or staff person. Disinterested members of the Board make a determination as to whether a conflict does exist and what subsequent action is needed.

The Coalitions Articles of Incorporation have been filed with the Indiana Secretary of State SOS and are available online at the SOSs website for viewing. Governing documents, a conflict of interest policy, and financial statements are available to the public upon request. IRS990/Desc0The Coalitions immunization program served the citizens of the State of Indiana by working to increase the immunization rates and decrease vaccine-preventable disease rates throughout the state. The Coalition provided professional development opportunities for immunization practitioners throughout the state by coordinating regional meetings sponsoring immunization summits collaborating on conferences to provide continuing education credits and presenting at professional meetings and conferences. The Coalition also hosted CHIRP summits and a new school nurse orientation, as well as continued social media campaigns to inform and educate the public about immunizations via their website, email marketing, PSAs, webcasts, and educational materials.

In 2014, funds were utilized to assist local health departments in becoming billing entities credential those health departments install billing software and train the health departments.

The equipment these health departments used was old, non-lab/pharmaceutical grade, and in need of replacement. This, combined with stricter CDC recommendations, necessitated upgraded vaccine storage and handling equipment. The Coalition used the grant monies to purchase and deliver new lab-grade vaccine refrigerators and freezers, as well as maintenance contracts on that equipment.
