Form 990, Part VI, Section B, Line 11

Hope community services inc. Has its form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reported is complete and accurate. When the form 990 has been prepared, reviewed by management and the audit committee and is ready to be filed with the internal revenue service, it is electronically sent to the board members of the organization for any comments. Any comments are then grouped, summarized and provided to the outside accountants. Each issue is documented and addressed until the return is finalized and approved for filing.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is applicable to all employees, board members, and officers. The conflicts are monitored by the executive director of the organization. If any potential or actual conflicts were to arise, the board member would be recused from voting. The executive director and the executive committee of the board would review the conflict. The resolution is determined on a case by case basis. Any violation of this policy will result in disciplinary action up to and including termination.

The executive director's salary is determined and approved by the board of directors based on a review of the salaries of similar positions at similar organizations. Increases are decided upon at board meetings and documented on the board minutes include the decision. The process for determining the executive director's salary was last undertaken in september 2014.

IT OPERATES FOUR MAJOR HOUSING PROGRAMS FUNDED THROUGH THE CITY OF NEW ROCHELLE, WESTCHESTER DEPARTMENT OF SOCIAL SERVICES, HOUSING URBAN DEVELOPMENT (HUD) AND NEW YORK STATE WHICH OFFERS CRITICAL HOUSING SUPPORT FOR CLIENTS. THE REFUGE IS A HOUSING PROGRAM SPECIFICALLY FOR LGBTQ YOUTH BETWEEN THE AGES OF 18 AND 25. IT IS TRANSITIONAL HOUSING, WHICH MEANS CLIENTS ARE PERMITTED TO STAY IN THE PROGRAM FOR A MAXIMUM OF 18 MONTHS. THE PROGRAM IS PERSON-CENTERED AND EACH CLIENT WILL HAVE A GOAL AND MULTIPLE CASEWORKERS WHO WILL ASSIST IN REACHING THAT GOAL. THE YOUTH WILL BE REFERRED FOR JOB TRAINING, VOCATIONAL PROGRAMS AND COMPLETION OF ANY HIGH SCHOOL OR COLLEGE. WE HAVE FORMED A PARTNERSHIP WITH WESTCHESTER JEWISH COMMUNITY SERVICES, CENTER LANE AND THE LOFT. THESE AGENCIES WILL PROVIDE BOTH INTENSIVE CASE MANAGEMENT AND SOCIAL INTERACTION SKILLS TRAINING FOR THE CLIENTS.CITY OF NEW ROCHELLE RE-HOUSING INITIATIVE (CNR) IS A PERMANENT HOUSING PROGRAM FOR ELIGIBLE HOMELESS RESIDENTS OF WESTCHESTER WHO HAVE SUBSTANCE ABUSE AND/OR MENTAL HEALTH DISABILITIES. THE PROGRAM BENEFITS INDIVIDUALS WHO ARE DEEMED TO BE HOUSING-READY WITH A FOCUS ON HOMELESS VETERANS. IN ADDITION TO PLACING THEM IN HOUSING THE PROGRAM PROVIDES CASE MANAGEMENT AND REFERRALS TO SUPPORTIVE SERVICES. IT IS FEDERALLY FUNDED THROUGH HUD.THE HOMELESS RESOURCE CENTER'S (HRC) GOAL IS TO REDUCE THE HOMELESS POPULATION IN NEW ROCHELLE. THIS PROGRAM WILL ASSIST THE HOMELESS POPULATION BY ENSURING THAT THEY HAVE A SAFE AND SECURE PLACE WHERE THEY CAN RECEIVE THE SERVICES THEY NEED. THROUGH A PARTNERSHIP WITH THE LOCAL HOMELESS DROP-IN CENTER CLIENTS ARE ASSURED OF SHELTER FROM THE ELEMENTS AND A PLACE TO SLEEP 24 HOURS PER DAY. DURING THE DAYTIME HOURS HOPE WILL PROVIDE SERVICES THAT INCLUDE COUNSELING, REFERRALS AND ONE TO ONE CASE MANAGEMENT. CLIENTS WILL ALSO BENEFIT FROM HOPE'S SOUP KITCHEN. HOPE'S COUNSELORS WILL EVALUATE AND LINK CLIENTS TO HOUSING AND OTHER THE SERVICES APPROPRIATE TO THEIR NEEDS. HRC WILL ALSO ASSIST CLIENTS IN THEIR APPLICATIONS FOR BENEFITS.

IN 2014 WE SERVED 36,630 MEALS TO FAMILIES WITH CHILDREN AND INDIVIDUAL CLIENTS. SOME OF THESE MEALS WERE EXTRA TAKE-OUT MEALS WE PROVIDE AT THE END OF EACH DINNER. WE ALSO SERVE SPECIAL HOLIDAY DINNERS ON THANKSGIVING, CHRISTMAS, EASTER, CINCO DE MAYO, MARTIN LUTHER KING DAY AND FOURTH OF JULY. OUR SOUP KITCHEN IS OUR PRIMARY OUTREACH TOOL TO CONNECT CLIENTS TO OUR OTHER PROGRAMS. PEOPLE WHO HAVE NO KITCHEN FACILITIES, LACK THE ABILITY TO COOK OR DO NOT HAVE FOOD TO PREPARE USE OUR SOUP KITCHEN AS THEIR MAIN SOURCE OF NUTRITION. OUR SOUP KITCHEN SERVES MANY PEOPLE WITH CHEMICAL DEPENDENCY AND MENTAL HEALTH ISSUES. WE ALSO SERVE DAY LABORERS, THE ELDERLY AND FAMILIES WITH CHILDREN.OUR SOUP KITCHEN SERVES AS A TOOL FOR HELPING RESIDENTS STRETCH THEIR FOOD DOLLARS AND THUS BE ABLE TO SECURE AND MAINTAIN PERMANENT HOUSING. OUR GOAL IS TO HELP LOW INCOME RESIDENTS MAINTAIN ADEQUATE NUTRITION AND PROVIDE A POINT OF ENTRY TO SUPPORTIVE SERVICES. THE SOUP KITCHEN ALSO PROVIDES A SAFE SPACE FOR HOMELESS CLIENTS AND RUN-AWAY YOUTH, AND PROVIDES A PLACE FOR OUR SENIOR CITIZENS WHO LIVE ALONE TO INTERACT.FOOD PANTRYOUR PANTRY DISTRIBUTES BAGS OF GROCERIES EVERY OTHER WEDNESDAY TO 1,062 OF NEW ROCHELLE'S LOW INCOME FAMILIES, HOMELESS INDIVIDUALS, SENIORS AND THE DISABLED. LAST YEAR OUR COMMUNITY OUTREACH/SELF-SUFFICIENCY PROGRAM ENGAGED 1490 LOW INCOME HOUSEHOLDS; HELPED 100 PEOPLE TO BECOME CITIZENS THROUGH OUR CITIZENSHIP CLASSES, HELD TWO SESSIONS PER MONTH OF THE MOTHERS' EMPOWERMENT GROUP. THROUGH COMMUNITY OUTREACH WE ADDED 15 NEW MEMBERS TO THE MOTHERS' GROUP, WE REFERRED 267 PEOPLE TO MEDICAL, MENTAL HEALTH OR SUBSTANCE ABUSE TREATMENT, AND WE HELPED 250 FAMILIES OBTAIN FOOD STAMPS. WE REFER ALL CLIENTS TO OUR SOUP KITCHEN AND FOOD PANTRY, AND IF NEEDED TO OUR HOUSING PROGRAM, THE LEARNING CENTER, BOCES, LEGAL SERVICES OF HUDSON VALLEY, ST. VINCENT'S HOSPITAL CRISIS INTERVENTION TEAM AND DEPARTMENT OF SOCIAL SERVICES. OUR GOAL IS TO PROVIDE A VENUE WHERE CLIENTS CAN COME TO SOLVE PROBLEMS OF DAILY LIVING, AND HELP LOW INCOME HOUSEHOLDS MOVE TOWARD SELF-SUFFICIENCY THROUGH JOB TRAINING AND EMPLOYMENT, PERMANENT HOUSING, EDUCATION, AND REFERRALS TO NEEDED SERVICES FROM BOTH PRIVATE AND GOVERNMENT AGENCIES.
