Form 990, Page 6, Part VI, Line 11B

The yearly audit draft is presented to the audit and compliance committee of the board. As part of this process, the committee addresses the independent auditors with no staff present. After having satisfactorily answered the committee's questions, the auditors issue final audited financial statements. The form 990 is prepared by the independent auditors based on information from the audited financial statements. The form 990 is reviewed by management and agreed with the independent auditors. The form is filed and the final copy of the audit and form 990 are posted on a document portal for all audit and compliance committee members to review.

All board members and employees are required on an annual basis and in addition anytime the policy is updated to review and sign the current policy.

The executive committee of the board of directors does a yearly evaluation of the executive director and upon compliance with goals set by the previous year and the data gathered the compensation offered to the executive director is determined. The executive director does the same for the associate executive director and brings that to the executive committee for their approval.

From a humble beginning in bay shore in 1985 as a provider of housing for persons living with mental illness, our housing programs now include the elderly, limited income families as well as persons with special needs. Mercy haven has grown throughout its 30 years to become a multifaceted community resource. We have brought services to both nassau and suffolk counties in new york and are dedicated members of the communities we serve. In addition to the housing programs described in part iii of this form, mercy haven operates the following programs: pleasant gardens adult home provides a home and personal care for 26 older adults who are unable to live independently. Pleasant gardens is licensed by new york state department of health and provides residents with services that include cooking, cleaning, laundry service, and medication management that are provided by staff who are on-site 24 hours per day, 7 days per week. Visits from a medical doctor and other specialists complete the care that is provided. The mercy advocacy program (map) provides free legal assistance to our residents on their civil legal emergencies. These emergencies often include threats to our residents' subsistence income (public assistance, ssi, ssd), their access to health care (medicare and medicaid), or their access to basic nutrition (food stamps). In 2014, map opened 110 cases bringing the total number of opened cases to 2,495 since the program's inception in 1997. Map also works for systemic change through its participation in class action suits on behalf of low-income people across new york state and continued work on 4 class actions suits that deal with the critical issues of access to nutrition and health care. Mercy haven's resident empowerment and achievement program (reap) seeks to provide residents with tools that will promote, motivate and encourage independent living as well as provide information about community resources available to them. In 2014, 34 different educational and social activities were offered and 83% of our residents participated in at least one activity. In addition, 56 residents participated in our supported employment program, which allowed them the opportunity for paid employment at mercy haven. Mercy enrichment is the center of mercy haven's community outreach efforts and includes the growing together community garden. In 2014 outreach efforts focused on both mental and physical health with participation in mental illness awareness week and hosting a free flu shot clinic for our residents and the public. The community garden generated 859 pounds of produce, bringing the garden's cumulative 3-year total to 3,176 pounds. During this 3 year period a total of 476 pounds of produce was donated to local food pantries. Breakthrough is mercy haven's outreach and education program offered to homeless and formerly homeless individuals. It consists of an intense 12- week, 24-session program that assists participants in understanding the factors that contributed to their homelessness and gives them the tools and confidence necessary to set and achieve personal goals that will prevent future homelessness. In 2014, breakthrough held 3 sessions, had 31 graduates bringing the total number of graduates to 189 since the program's inception in 2007.
