Mission and Program Overview

Mission

Since 1991, the maine migrant health program (mmhp) has championed the health care needs of migrant and seasonal farmworkers (msfws) and their families by providing quality primary, preventive, and chronic health care services to msfws across the state. Msfws are one of the nation's most vulnerable and invisible populations. Their health status is compromised by poverty, living/working conditions, immigration status, interruptions in care, lack of culturally and linguistically appropriate services (clas), social isolation, and an overwhelming lack of access to basic health care.. As a migrant health voucher program, the mmhp is dedicated to maximizing access to health care for maine's seasonal workers by providing mobile medical care, and by reimbursement arrangements (i.e. Vouchers) with a broad network of medical, dental, vision and behavioral health providers.

To improve the health status of maine's seasonal workers and their families by providing culturally appropriate care and services.

Governance Explanations

Form 990, Page 6, Part VI, Line 11B

A draft form 990, including all supplemental forms, is distributed to all board members approximately one week prior to the scheduled meeting. The finance committee meets prior to the board meeting, reviews the 990, and makes its recommendations to the full board. Board members then discuss the committee's recommendation and vote. If approved, either as presented or as amended based on their discussion, staff is then authorized to submit all forms on its behalf. If not approved, staff resolves problems and resubmits for the next meeting.

Mmhp has a conflict of interest policy that applies to personnel. Mmhp's by-laws include a conflict of interest section for board and advisory board members which call for annual disclosure of potential conflicts. A conflict of interest disclosure form is required to be completed by all board members on an annual basis.

Form 990, Page 6, Part VI, Line 15A

Mmhp's board of directors determines all compensation, including salary adjustments and one-time cost of living adjustments, for the executive director. Information is generally provided to board members for comparable organizations of similar size and operations for comparison and discussion purposes. All issues are discussed at the board table in the absence of any staff members and voted on for documentation in the board minutes. All other staff compensation remains the purview of the executive director as it falls within the parameters of the agency's annual budget, which is also subject to approval by the board.

Maine migrant health program, as a federally-funded nonprofit corporation, is required to have an annual audit by an outside accounting firm. The audited financial statements and any letters to management, in conjunction with the federal financial report (standard form 425) are subsequently filed with hrsa, and thus available to any public inquiry. In addition, all governing documents, financial statements, and tax returns are available upon request to mmhp's administrative headquarters.

Both washington and aroostook qualify as "very rural", their population density being 14.0/sq m and 11.6/sq m, resp. Aroostook county has the distinct honor of being the only county east of the mississippi that qualifies as a frontier designated area. In addition, parts of these major harvest areas have been classified as health professional shortage areas, medically underserved areas, or as having a medically underserved population. Unfortunately, they are home to mental health and dental shortage areas as well. Maine state demographics only add to the difficulties msfws face. Add to this inadequate transportation, geographic isolation, unfamiliarity with local resources and systems, as well as the inability to arrange time off from work. These are the barriers mmhp's community services are designed to bridge and/or eliminate, thereby increasing access to care. These community services are accomplished through outreach services including health education, care coordination, prescription assistance, transportation, eligibility assistance, medical interpretation, and referral services to local network providers. Staff also spend time scouting for new crews and workers to ensure all who need services have the opportunity to access them. Thus, community services are an integral component in mmhp's success in meeting migrant and seasonal farmworker needs and in fulfilling its mission to improve the health status of maine's seasonal workers and their families.

Mobile medical units - as discussed, part of maine migrant's unique model of service delivery is direct delivery of care at worker camps, in communities, and at work sites. To accomplish this, mmhp operates a fleet of vehicles designed and utilized as mobile medical centers. These mobile medical units (mmus) are staffed by clinical staff and bilingual staff who ensure that clinics run smoothly and efficiently and that farmworker needs are identified correctly and treated appropriately contracted interpretation services are utilized when worker languages are not spoken by staff. Clinics are generally scheduled after harvest work hours. When necessary, arrangements are made and assistance provided for any referrals to local network voucher providers. Expenses 94,959 including grants of 0. Revenue 0 behavioral health services - due to an overwhelming declaration of need by migrant and seasonal farmworkers, behavioral health services were integrated into mmhp's core services in 2009. These services work hand-in- hand to support and enhance the provision of quality primary, preventive, and chronic health care services to seasonal workers across the state. Behavioral services currently include mental health and substance abuse counseling with referral for treatment. . Line 2c, financial oversight - mmhp's board of directors, with guidance from the finance committee and input from management, is responsible for fiscal management of the organization and approval of the selection of an independent accountant.

The program has been granted exemption from federal income tax under section 501(c)(3) of the internal revenue code. In addition, the program qualifies for the charitable contribution deduction under section 170(b) (1)(a) and has been classified as an organization that is not a private foundation under section 509(a)(2). Management evaluated the program's tax positions and concluded that the program had taken no uncertain tax positions that required adjustment to the financial statements. When necessary, the program accounts for interest and penalties related to uncertain tax positions as part of its provision for federal and state income taxes. The program does not expect that unrecognized tax benefits arising from tax positions will change significantly within the next twelve months. The program is currently open to audit under the statute of limitations by the internal revenue service and state taxing authorities for the years ending march 31, 2015 through 2017.

MMHP'S EXISTING SERVICE AREA IS THE ENTIRE STATE OF MAINE (33,215 SQ M) WITH MAJOR HARVEST AREAS IN AROOSTOOK, HANCOCK, WASHINGTON, AND ANDROSCOGGIN COUNTIES. BOTH WASHINGTON AND AROOSTOOK QUALIFY AS "VERY RURAL", THEIR POPULATION DENSITY BEING 14.0/SQ M AND 11.6/SQ M, RESP. MSFWS ARE ONE OF THE NATION'S MOST VULNERABLE AND INVISIBLE POPULATIONS. THEIR HEALTH STATUS IS COMPROMISED BY POVERTY, LIVING/WORKING CONDITIONS, IMMIGRATION STATUS, INTERRUPTIONS IN CARE, LACK OF CULTURALLY AND LINGUISTICALLY APPROPRIATE SERVICES (CLAS), SOCIAL ISOLATION, AND AN OVERWHELMING LACK OF ACCESS TO BASIC HEALTH CARE.. AS A MIGRANT HEALTH VOUCHER PROGRAM, THE MMHP IS DEDICATED TO MAXIMIZING ACCESS TO HEALTH CARE FOR MAINE'S SEASONAL WORKERS BY PROVIDING MOBILE MEDICAL CARE, AND BY REIMBURSEMENT ARRANGEMENTS (I.E. MANAGEMENT EVALUATED THE PROGRAM'S TAX POSITIONS AND CONCLUDED THAT THE PROGRAM HAD TAKEN NO UNCERTAIN TAX POSITIONS THAT REQUIRED ADJUSTMENT TO THE FINANCIAL STATEMENTS. WHEN NECESSARY, THE PROGRAM ACCOUNTS FOR INTEREST AND PENALTIES RELATED TO UNCERTAIN TAX POSITIONS AS PART OF ITS PROVISION FOR FEDERAL AND STATE INCOME TAXES. THE PROGRAM DOES NOT EXPECT THAT UNRECOGNIZED TAX BENEFITS ARISING FROM TAX POSITIONS WILL CHANGE SIGNIFICANTLY WITHIN THE NEXT TWELVE MONTHS. MSFWS ARE ONE OF THE NATION'S MOST VULNERABLE AND INVISIBLE POPULATIONS. THEIR HEALTH STATUS IS COMPROMISED BY POVERTY, LIVING/WORKING CONDITIONS, IMMIGRATION STATUS, INTERRUPTIONS IN CARE, LACK OF CULTURALLY AND LINGUISTICALLY APPROPRIATE SERVICES (CLAS), SOCIAL ISOLATION, AND AN OVERWHELMING LACK OF ACCESS TO BASIC HEALTH CARE.. AS A MIGRANT HEALTH VOUCHER PROGRAM, THE MMHP IS DEDICATED TO MAXIMIZING ACCESS TO HEALTH CARE FOR MAINE'S SEASONAL WORKERS BY PROVIDING MOBILE MEDICAL CARE, AND BY REIMBURSEMENT ARRANGEMENTS (I.E. VOUCHERS) WITH A BROAD NETWORK OF MEDICAL, DENTAL, VISION AND BEHAVIORAL HEALTH PROVIDERS.

IN ADDITION, PARTS OF THESE MAJOR HARVEST AREAS HAVE BEEN CLASSIFIED AS HEALTH PROFESSIONAL SHORTAGE AREAS, MEDICALLY UNDERSERVED AREAS, OR AS HAVING A MEDICALLY UNDERSERVED POPULATION. UNFORTUNATELY, THEY ARE HOME TO MENTAL HEALTH AND DENTAL SHORTAGE AREAS AS WELL. MAINE STATE DEMOGRAPHICS ONLY ADD TO THE DIFFICULTIES MSFWS FACE. ADD TO THIS INADEQUATE TRANSPORTATION, GEOGRAPHIC ISOLATION, UNFAMILIARITY WITH LOCAL RESOURCES AND SYSTEMS, AS WELL AS THE INABILITY TO ARRANGE TIME OFF FROM WORK. THESE ARE THE BARRIERS MMHP'S COMMUNITY SERVICES ARE DESIGNED TO BRIDGE AND/OR ELIMINATE, THEREBY INCREASING ACCESS TO CARE. THESE COMMUNITY SERVICES ARE ACCOMPLISHED THROUGH OUTREACH SERVICES INCLUDING HEALTH EDUCATION, CARE COORDINATION, PRESCRIPTION ASSISTANCE, TRANSPORTATION, ELIGIBILITY ASSISTANCE, MEDICAL INTERPRETATION, AND REFERRAL SERVICES TO LOCAL NETWORK PROVIDERS. STAFF ALSO SPEND TIME SCOUTING FOR NEW CREWS AND WORKERS TO ENSURE ALL WHO NEED SERVICES HAVE THE OPPORTUNITY TO ACCESS THEM. THUS, COMMUNITY SERVICES ARE AN INTEGRAL COMPONENT IN MMHP'S SUCCESS IN MEETING MIGRANT AND SEASONAL FARMWORKER NEEDS AND IN FULFILLING ITS MISSION TO IMPROVE THE HEALTH STATUS OF MAINE'S SEASONAL WORKERS AND THEIR FAMILIES.

CLINICS ARE GENERALLY SCHEDULED AFTER HARVEST WORK HOURS. WHEN NECESSARY, ARRANGEMENTS ARE MADE AND ASSISTANCE PROVIDED FOR ANY REFERRALS TO LOCAL NETWORK VOUCHER PROVIDERS. EXPENSES 94,959 INCLUDING GRANTS OF 0. REVENUE 0 BEHAVIORAL HEALTH SERVICES - DUE TO AN OVERWHELMING DECLARATION OF NEED BY MIGRANT AND SEASONAL FARMWORKERS, BEHAVIORAL HEALTH SERVICES WERE INTEGRATED INTO MMHP'S CORE SERVICES IN 2009. THESE SERVICES WORK HAND-IN- HAND TO SUPPORT AND ENHANCE THE PROVISION OF QUALITY PRIMARY, PREVENTIVE, AND CHRONIC HEALTH CARE SERVICES TO SEASONAL WORKERS ACROSS THE STATE.
