Mission and Program Overview

Mission

Since 1991, the Maine Migrant Health Program (MMHP) has championed the health care needs of migrant and seasonal farmworkers (MSFWs) and their families. The organization provides quality primary, preventive, and chronic health care services to MSFWs across the state. MSFWs are one of the nation's most vulnerable and invisible populations. Their health status is compromised by poverty, living and working conditions, immigration status, interruptions in care, lack of culturally and linguistically appropriate services (CLAS), social isolation, and an overwhelming lack of access to health care in general. As a migrant health voucher program, the MMHP is dedicated to maximizing access to health care for Maine's MSFWs by providing mobile medical care, and by reimbursement arrangements (i.e. vouchers) with a broad network of medical, dental, and behavioral health providers.

To improve the health status of migrant and seasonal farmworkers and their families by providing culturally appropriate care and services.

Governance Explanations

Form 990, Part VI, Section B, line 11

A draft Form 990, including all supplemental forms, is distributed to all Board members with other meeting materials approximately one week prior to the scheduled Board meeting date. Information contained in the forms is discussed during the meeting with any requested changes noted for the final draft. Board members then vote to approve the filing as presented, or as amended if changes are required, and authorize staff to submit all forms on its behalf. Staff subsequently follows up with the auditors to complete and submit the Form 990 as directed and approved by the Board of Directors.

Form 990, Part VI, Section B, line 12C

MMHP had a long-standing conflict of interest policy that applied equally to personnel and members of the Board of Directors and advisory boards. During FY11, MMHP amended this policy to apply to personnel only. At the same time, MMHP amended its by-laws to expand the conflict of interest section for Board and advisory board members and require annual disclosure of potential conflicts. A conflict of interest disclosure form was created and is now required to be completed on an annual basis.

Form 990, Part VI, Section B, line 15A

MMHP'S Board of Directors determines all compensation, including salary adjustments for the Executive Director. Information is generally provided to Board members for comparable organizations of similar size and operations for comparison and discussion purposes. All issues are discussed at the Board table in the absence of any staff members and voted on for documentation in the Board minutes. All other staff compensation remains the purview of the Executive Director as it falls within the parameters of the agency's annual budget, which is also subject to approval by the Board.

Form 990, Part VI, Section C, line 19

Maine Migrant Health Program, as a federally-funded nonprofit corporation, is required to have an annual audit by an outside accounting firm. The audited financial statements and any letters to Management, in conjunction with the Federal Financial Report (standard form 425), are subsequently filed with HRSA, and thus available to any public inquiry. In addition, all governing documents, financial statements, and tax returns are available upon request to MMHP's administrative headquarters.

IRS990/Desc0Community Service (Enabling Services) - The Maine Migrant Health Program is a 330g migrant voucher program. As such, it does not operate out of a fixed site or "bricks and mortar" health center. All direct services are provided in the community, so the engagement of patients and clients is always from an 'outreach' perspective. Mobile medical clinics, pesticide safety trainings, health education sessions, etc, all take place in the farmworkers' communities - their camps, homes, work places, Rakers' Center etc. MMHP's enabling services are provided where they are most needed, where farmworkers live and work. These services are the cornerstone of migrant voucher programs across the country and provide a strong base of supportive services that create access. MMHP's existing service area is the entire state of Maine (33,215 sq mi) with major harvest areas in Aroostook, Hancock, Washington, and Androscoggin counties. Both Washington and Aroostook qualify as 'very rural', their population density being 14.0/sq mi and 11.6/sq mi, respectively. Aroostook County also has the distinct honor of being the only county east of the Mississippi that qualifies as a frontier designated area. In addition, parts of these major harvest areas have been classified as Health Professional Shortage Areas, Medically Underserved Areas, or as having a Medically Underserved Population. Unfortunately, they are home to mental health and dental shortage areas as well. Maine state demographics only add to the difficulties MSFWs face as described in Part III, Line 1. Add to this inadequate transportation, geographic isolation, unfamiliarity with local resources and systems, as well as the inability to arrange time off from work. These are the barriers community (enabling) services are designed to bridge and/or eliminate, thereby increasing access to care. This is accomplished through outreach services, health education, case management, transportation, eligibility assistance, medical interpretation, and referral services to local network providers. Community service workers also spend time scouting for new crews and workers to ensure all who need services have the opportunity to access them. Thus, enabling services are an integral component in MMHP's success in meeting migrant and seasonal farmworker needs and in fulfilling its mission to improve the health status of MSFWs and their families.

IRS990/MissionDesc0Since 1991, the Maine Migrant Health Program (MMHP) has championed the health care needs of migrant and seasonal farmworkers (MSFWs) and their families. The organization provides quality primary, preventive, and chronic health care services to MSFWs across the state. MSFWs are one of the nation's most vulnerable and invisible populations. Their health status is compromised by poverty, living and working conditions, immigration status, interruptions in care, lack of culturally and linguistically appropriate services (CLAS), social isolation, and an overwhelming lack of access to health care in general. As a migrant health voucher program, the MMHP is dedicated to maximizing access to health care for Maine's MSFWs by providing mobile medical care, and by reimbursement arrangements (i.e. vouchers) with a broad network of medical, dental, and behavioral health providers.

IRS990/ProgSrvcAccomActy3Grp/Desc0Mobile Medical Units - As discussed, part of Maine Migrant's unique model of service delivery is to go directly to the migrant and seasonal farmworker camps, communities, and work sites. To accomplish this, MMHP operates a fleet of vehicles designed and utilized as mobile medical centers. These mobile medical units (MMUs) are staffed by, at a minimum, one provider and two outreach and/or intake workers. At least one member of the team is bilingual to ensure that the clinics run smoothly and efficiently, and that farmworker needs are identified correctly and treated appropriately. Team members are either on staff, under contract, or working as volunteers. Clinics are generally scheduled after harvest work hours and, whenever possible, continue until all workers have been seen by a provider. When necessary, arrangements are made and assistance provided for any referrals to local voucher network providers.

IRS990/ProgSrvcAccomActyOtherGrp/Desc0Dental and Pharmacy Services - Dental and pharmacy services are both provided solely under voucher arrangement as MMHP's mobile medical units (MMUs) are not equipped or staffed to provide them directly on site. A number of dentists and pharmacies are included in MMHP's provider network in the communities where the migrant and seasonal farmworkers live and work. This represents a return to Maine Migrant's original delivery model for these services. For a short period of time, MMHP was able to contract to provide preventive dental services on site on a limited basis. This service was much-needed and well-received by the farmworkers, but unfortunately MMHP was unable to secure these services during the past two years. Providing preventive dental care on site remains a high priority and it is anticipated this service will be re-established in the upcoming harvest season.
IRS990/ProgSrvcAccomActyOtherGrp/Desc1Behavioral Health Services - Due to an overwhelming declaration of need by migrant and seasonal farmworkers, behavioral health services were added to MMHP's core services six years ago. These services have been successfully integrated into MMHP's primary care model and work hand in hand to support and enhance the provision of quality primary, preventive, and chronic health care services to MSFWs across the state. Behavioral services currently include mental health and substance abuse counseling. Care is provided directly at migrant camps and work sites on the mobile medical units by MMHP's behavioral health specialist, as well as through voucher (referral) providers.
