The vp of administration and director of finance reviews form 990. Form 990 is then presented to the audit committee for approval. Form 990 then goes to full board for approval. Form 990 is then filed.

Annually, the board reviews its conflict of interest policy. All board members report any possible conflicts of interest and excuse themselves from voting if there is a possibility of an appearance of a conflict of interest.

The president/ceo is selected by the board of directors with the compensation committee determining fair compensation. The organization utilizes annual performance reviews along with comparable data from salary surveys for all positions, including the president/ceo. Compensation of the vice presidents was determined by the president/ceo in consultation with the board chair based on performance evaluation and salary surveys. This process is conducted annually and is documented in the executive committee minutes. The executive committee has the authority to act on behalf of the board.

Teaching and learning centers ("tlc") shut down in march 2020 due to the pandemic providing telehealth services until reopening at a reduced capacity in january 2021. The kupuna wellness centers also shutdown in march 2020 due to the pandemic. In its place 3 new services were launched - the green bag, kupuna tech, and kupuna connect - that provide safe access to local fruits and veggies, make learning to use technology approachable, and encourage socialization from the safety of home.

PART X, LINE 2:

U.s. Gaap requires uncertain tax positions to be recognized in the consolidated financial statements if they are more likely than not to fail upon regulatory examination. Management has evaluated lp and lpf's tax positions as of june 30, 2021 and 2020 and for the years then ended, and determined that lp and lpf had no uncertain tax positions required to be reported in accordance with u.s. Gaap. Lp and lpf are subject to routine audits by taxing jurisdictions; however, there are currently no audits in progress for any open tax periods.

LANAKILA MEALS ON WHEELS IS HAWAII'S LARGEST AND ONLY ISLAND-WIDE MEAL PROVIDER FOR SENIORS AND INDIVIDUALS WITH DISABILITIES. IN FISCAL YEAR 2021, LANAKILA MEALS ON WHEELS PROVIDED OVER 576,000 MEALS THROUGH OUR HOME AND COMMUNITY-BASED SERVICES. THE GOAL OF LANAKILA MEALS ON WHEELS IS TO ALLOW SENIORS AND INDIVIDUALS WITH DISABILITIES TO AGE IN PLACE WITH DIGNITY BY PREVENTING HUNGER AND SOCIAL ISOLATION WHICH OFTEN LEADS TO INSTITUTIONALIZATION.

THE PROGRAM OFFERS A VARIETY OF CLASSES FOCUSED ON DEVELOPING KEY SOCIAL AND LIFE SKILLS AND PROMOTING COMMUNITY INCLUSION AND INDEPENDENCE. IN FISCAL YEAR 2021, TLC SERVED NEARLY 120 PARTICIPANTS. ACTIVITY AREAS INCLUDE PERSONAL DEVELOPMENT, EDUCATION, HOME MANAGEMENT, COMMUNITY EXPLORATION, RECREATION AND LEISURE, AND PREVOCATIONAL READINESS. IN MARCH 2020 ALL IN-PERSON SERVICES WERE SUSPENDED DUE TO COVID-19. THE TRANSITION WAS MADE TO OFFER TELEHEALTH SERVICES. IN PERSON SERVICES HAVE SLOWLY RE-OPENED, AS ALLOWABLE UNDER STATE AND FUNDER GUIDELINES. TELEHEALTH ALSO CONTINUES TO BE OFFERED AS APPROPRIATE.
