Annual Performance and Compensation Review

990 part vi line 15 doheny eye institute conducts an annual performance and compensation review. This was last conducted in june, 2016. The institute conducted comparison for management positions using the non-profit "guidestar compensation report, fifteen edition - september, 2015." any proposed salary adjustment for the executive director is reviewed and approved by the doheny eye institute board of directors prior to taking effect.

Form 990 Review

990 part vi line 11a the tax preparer emails a draft copy of form 990 to the organization. The draft is reviewed by the organization's director of finance, chief financial officer, and finance committee. Comments and questions are exchanged and resolved both within the organization and between the organization and the tax preparer. If necessary, a second draft is circulated prior to issuing the final form 990 to be filed.

990 PART VI LINE 19 The Organization's governing documents, conflict of interest policy, and recent federal income tax returns are made available for public inspection at the Organization's principal office during normal business hours.

Conflict of Interest Policy

990 PART VI LINE 12 Officers, directors, and key employees are required to complete and submit a conflict of interest questionnaire annually. The forms are tracked and reviewed by members of the conflict of interest committee. Should conflicts arise mid-year, according to the policy, the officer, director, or key employee would immediately be required to: (i) disclose the existence and circumstances of any transaction or relationship that could give rise to an actual or apparent conflict of interest with DEI, (ii) refrain from using personal influence to encourage DEI to take an action or decline to take an action involving such a transaction or relationship and (iii) recuse himself or herself from participation in any discussions or decisions regarding such transaction or relationship with directors, officers or key employees of DEI. This includes recusal during those portions of Board, Executive Committee or other Board committee meetings in which matters pertaining to such a transaction or relationship are being discussed, except to respond to requests for information about the transaction or relationship. Should any suspected or reported violations be found, the Chairman of the Board of Directors would promptly be notified and would then recommend appropriate action to the Executive Committee or the Board.

SCH D PART X LINE 2 The Institute is a not-for-profit California corporation, granted tax-exempt status under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the "Code"), the Institute is also exempt from California taxes accordingly, no provision has been made for income taxes. The Institute has analyzed its tax positions taken for filings with the Internal Revenue Service and State of California. It believes that its tax filing positions will be sustained upon examination and does not anticipate any adjustments that would result in a material adverse affect on its financial condition, results of operations or cash flows. United States federal and State of California returns have statutes of limitations that generally range from three to four years. No open tax years are currently under examination.

TO INCREASE KNOWLEDGE OF THE HUMAN EYE, ITS CONDITIONS AND DISEASES, THROUGH RESEARCH; 2. TO TRANSMIT THIS KNOWLEDGE THROUGH EDUCATION, TRAINING, AND COMMUNITY SERVICE; AND 3. TO FACILITATE THE APPLICATION OF ITS RESEARCH TO IMPROVE PATIENT CARE AND FOSTER FURTHER RESEARCH. RESEARCH ACTIVITIES FOR THE PERIOD AND SELECTED FOR LONG-TERM FOCUS INCLUDE: RETINAL AND MACULAR DEGENERATION RESEARCH THROUGH THE STUDY OF RETINAL PIGMENT EPITHELIUM, RETINAL CELL TRANSPLANTATION, MOLECULAR GENETICS OF INHERITED DISEASES OF THE RETINA AND GENE THERAPY FOR RETINAL DISORDERS; MODIFICATION OF CORNEAL SHAPE AND MORPHOLOGY THROUGH CORNEAL RESEARCH AND REFRACTIVE SURGERY INNOVATIONS; AND REGENERATION OF DAMAGED OPTIC NERVE. On December 18, 2013, the Doheny Eye Institute executed an agreement with the University of California at Los Angeles (UCLA) for a 99 year affiliation. The purpose of this affiliation is to carry out the Institute's mission -- to further the conservation, improvement, and restoration of human eyesight -- through research, education, and clinical care (clinical care is provided directly by UCLA). Both the Institute and UCLA-Stein Eye Institute are dedicated to providing the public with the highest quality of ophthalmic care and the most innovative and strategic research for the improvement of human eyesight. DURING THE YEAR, DEI PUBLISHED 114 ARTICLES, 40 CONFERENCE ABSTRACTS, AND PRODUCED 115 SCIENTIFIC PRESENTATIONS AND PROTOCOLS.
