Activity 2

For the year june 30, 2012, foothill family service provided mental health treatment and early intervention services providing early esteem services to 553 children 0-5 and their parents at preschools and head start preschools in 11 school disctricts. Early head start provided weekly home visits to families of children 0-3 and pregnant women using an evidenced based curriculum that focuses on child development, provided case managment services and bi-weekly socialization groups. Foothill family's early head start program serves el monte, south el monte and parts of la puente; we were awarded 125 slots for children 0-3. School readiness initiative (sri) in collaboration with azusa unified school disctrict provided in home visits, parenting workshops and case management services to 89 children and 121 parents on the azusa community.

Activity 3

For the year ended june 30, 2012, foothill family service provided service to 1,470 teens, their babies and families. In total teen family service had 79 graduates of which 43% plan to attend a two year college, 18% plan to enroll in a trade or vocational school, 4% enrolled in a four year college and 2% will enter the workforce.
