The Form 990 is reviewed by the CEO, COO, and CFO and upon review, presented to the Finance Committee of the Board of Directors for their review and approval.

Form 990, Part VI, Section B, line 12C

The Organization's conflict of interest policy covers the Board of Directors, management and all employees. All reports are submitted to the compliance team, which reviews potential conflicts and determines whether a conflict exists. Any individual with a conflict is excluded from all Board deliberations regarding the issue. An annual report is given to the President of the Board summarizing any allegations, investigations, complaints, losses, theft, fraud within the past 12 months and the corrective actions taken.

The Organization contacted and included many sources in their compensation determination process. One of those sources did specific research based on the size of the Organization, number of employees, scope of responsibility, and numerous other criteria. There were several executive compensation comparisons that matched the corporation and detailed research. The research provided determined the salary increase appropriate for the location and responsibilities of their CEO.

AP&H, Inc. is registered as a not-for-profit public benefit corporation and is exempt from income taxes under section 501(c)(2) of the Internal Revenue Code. MHSA is incorporated in the state of California as a limited liability company and files its tax returns on the accrual basis of accounting. Anka has evaluated its current tax positions and has concluded that as of June 30, 2016, Anka does not have any significant uncertain tax positions for which a reserve would be necessary.

IRS990/Desc0CONTRA COSTA COUNTY:These are contracts funded by Contra Costa County to either provide an alternative to hospitalization for clients experiencing a sub-acute mental health crisis, provide housing and community integration for clients with mental illness or provide employment planning and development for clients with mental illness. The residential programs provide a home-like environment where intensive therapy and medication management can be provided. Two residential programs for adults served 1,533 clients this year, two outpatient programs served 95 individuals each month, one shelter that served 20 homeless each night, two community integration locations that served 162 homeless each day and a rehabilitation center that served 46 clients each day. Stays at the residential programs can vary from seven days to three months.
