Governance Explanations

Form 990, Part VI, Section B, Line 11B

Policy: chs believes strongly in providing transparent information to the public and therefore, will provide accurate and complete information regarding the agency's tax-exempt status to the irs and other entities as indicated. The filing of the 990 is an example of the information chs will share with the public. Prior to submission of the 990, the document will be reviewed by the board of directors. Procedures: annually, the finance director will work with the agency's accounting firm to complete the form 990 and file the document by the deadline. All information about the agency's mission, activities, etc will be developed by the executive director or his/her designee. The final draft of this information will be reviewed by the executive director prior to the information being entered on the form. Once the form 990 has been completed, the board of directors finance committee will be responsible for reviewing the form under the direction of the treasurer. Once reviewed, the committee will approve submission of the form. The treasurer will advise the board president and officers of any concerns, etc. And of the approval for submission. Depending on the submission due date, the treasurer will provide a report at the executive committee or full board meeting. The finance director will assist in scheduling any and all meetings. The finance director will coordinate the submission of the form 990 by either mail or electronic means. The finance director will retain proof of the mailing or electronic submission. A file will be established electronically and in hard copy of the 990 filings.

Form 990, Part VI, Section B, Line 12C

Center for human services (chs) has specific written policies for monitoring potential or actual conflicts of interest. Persons covered under the policy include any employee or volunteer with decision-making authority (board members). For board members, the executive committee of the board determines whether a conflict exists. For employees, the executive director makes the final decision. Employees update their conflict of interest disclosure statement annually as do board members. Actual conflicts are reviewed by the board of directors executive committee for volunteers and by the executive director and members of the senior management team for employees. The board executive committee reviews any actual or potential conflicts involving the executive director. Certain restrictions may be applied depending on the actual conflict identified for both volunteers and employees. Restrictions could include prohibiting participation in board deliberations and transactions and for employees, prohibiting participation at identified meetings, etc.

Form 990, Part VI, Section B, Line 15

The executive director's compensation is reviewed by members of the board who belong to either the executive, finance, or governance committees. A request for salary information from other local non profit agencies is sent out annually in survey format. A record of the deliberations and decision regarding the executive director's compensation is contained in the board finance committee minutes. A similar process is used to determine salaries for key employees.

THE GOAL OF THE PROGRAM IS TO ASSIST WOMEN IN MAINTAINING DRUG AND ALCOHOL FREE LIFESTYLES AND TO DEVELOP POSITIVE PARENTING SKILLS. CLIENTS LEARN TO IDENTIFY AND IMPLEMENT HEALTHY INTERPERSONAL RELATIONSHIPS AND TO PROVIDE SAFE, SOBER, AND NURTURING HOME ENVIRONMENTS FOR THEMSELVES AND THEIR CHILDREN. 100% OF THE WOMEN PARTICIPATING IN THE FIRST STEP PROGRAM REPORTED THAT THEIR RELATIONSHIP WITH THEIR CHILD HAD IMPROVED. STUDENT ASSISTANCE PROGRAM (SAP) PROVIDES SHORT-TERM EDUCATION, PREVENTION, AND INTERVENTION SERVICES TO STUDENTS K-12 BY BUILDING LIFE, SOCIAL, AND COPING SKILLS. OUR TOP FIVE CONTACT REASONS IN THIS PROGRAM RELATE TO ANGER MANAGEMENT, ACADEMIC, ANXIETY, FAMILY, AND RELATIONSHIP ISSUES. DURING 2021-2022 THE SAP PROGRAM SERVED 5,617 STUDENTS. SCHOOL STAFF ARE SURVEYED AND CONSISTENTLY REPORT THAT REFERRED STUDENTS BENEFIT FROM SAP SERVICES.

CURRENT FRC SERVICES INCLUDE COMMUNITY OUTREACH AND ENGAGEMENT ACTIVITIES, STRENGTH BASED FAMILY ASSESSMENTS, CASE MANAGEMENT, AND MENTAL HEALTH COUNSELING, PARENT EDUCATION AND SKILL BUILDING CLASSES, ASSISTANCE WITH TRANSLATION AND APPLICATION FOR AID AND INSURANCE, TRANSPORTATION TO APPOINTMENTS, AND LINKAGE TO OTHER RESOURCES AS DEEMED APPROPRIATE. ADDITIONALLY, INDIVIDUALS AND FAMILIES HAVE ACCESS TO OTHER PUBLIC/PRIVATE SERVICES CO-LOCATED AT THE SITES. IN ADDITION TO THE SERVICES LISTED ABOVE, OUR FAMILY RESOURCE CENTERS PROVIDED EMERGENCY FOOD/CRISIS ASSISTANCE TO 11,528 PEOPLE DURING 2021/2022.

YOUTH NAVIGATION CENTER SERVICES INCLUDE: HUTTON HOUSE, A TEMPORARY SHELTER FOR RUNAWAYS, HOMELESS, AND YOUTH IN CRISIS (AGES 13-17), PROVIDES SERVICES IN A RESIDENTIAL SETTING FOR 8 YOUTH AT A TIME FOR A MAXIMUM OF 90 DAYS. IN ADDITION, DAY SERVICES ARE AVAILABLE FOR YOUTH AND THEIR FAMILIES AND THE CRISIS LINE IS AVAILABLE 24 HOURS A DAY. DURING FISCAL YEAR 2021-2022, 58 YOUTH RECEIVED RESIDENTIAL SERVICES AT HUTTON HOUSE AND AN ADDITIONAL 108 YOUTH WERE SERVED ON A NON-RESIDENTIAL BASIS. 60% OF ALL YOUTH RECEIVING SERVICES AT HUTTON HOUSE REPORTED THEY HAD BEEN TAUGHT SKILLS TO USE WHEN CONFLICT HAPPENS AT HOME. LOW BARRIER YOUTH SHELTER, A NEW LOW BARRIER EMERGENCY SHELTER FOR YOUTH EXPERIENCING HOMELESSNESS (AGES 18-24), PROVIDES EMERGENCY SHELTER FOR 24 YOUTH AT A TIME FOR A MAXIMUM OF 90 DAYS. THE LOW BARRIER YOUTH SHELTER BEGAN OPERATION IN OCTOBER, 2021. IN THE FIRST YEAR OF THE LOW BARRIER YOUTH SHELTER, 82 YOUTH WERE SHELTERED AT THE LOW BARRIER YOUTH SHELTER. 23% OF THOSE YOUTH EXITED TO TRANSITIONAL HOUSING, AND 1% EXITED TO PERMANENT HOUSING. 69% OF YOUTH SURVEYED FELT SAFE AT THE LOW BARRIER YOUTH SHELTER. PATHWAYS, A TRANSITIONAL LIVING & SUPPORTIVE SERVICES PROGRAM FOR 16 YOUNG ADULTS (AGES 18-24), SERVES HOMELESS YOUTH. IN ADDITION TO HOUSING, SERVICES INCLUDE LIFE SKILLS AND EMPLOYMENT TRAINING, CASE MANAGEMENT, MENTAL HEALTH AND SUBSTANCE ABUSE COUNSELING, AND SUPPORT IN REACHING EDUCATIONAL GOALS. DURING FISCAL YEAR 2021-2022, PATHWAYS SERVED 37 RESIDENTIAL PARTICIPANTS AND 28% OF THE YOUNG ADULTS LIVING AT PATHWAYS INCREASED THEIR INCOME.
