Governance Explanations

Form 990, Part VI, Line 11B: Form 990 Review Process

Policy: CHS believes strongly in providing transparent information to the public and therefore, will provide accurate and complete information regarding the agency's tax-exempt status to the IRS and other entities as indicated. The filing of the 990 is an example of the information CHS will share with the public. Prior to submission of the 990, the document will be reviewed by the Board of Directors.Procedures: Annually, the Director-Finance will work with the agency's accounting firm to complete the 990 form and file the document by the deadline date. The Director-Finance will be responsible for coordinating completion of the form. All information describing the agency under the summary section describing the agency mission, activities, etc. will be developed by the Executive Director or his/her designee. The final draft of this information will be reviewed by the Executive Director prior to the information being entered on the form. Once the 990 form has been completed the Board of Director's Finance Committee will be responsible for reviewing the form under the direction of the Board of Director's Treasurer. Once reviewed, the Committee will approve submission of the form. The Board of Director's Treasurer will advise the Board President and officers of any concerns, etc. and of the approval for submission. Depending on the submission due date, the Treasurer will provide a report at the Executive Committee or full Board meeting. The Director-Finance will assist in scheduling any and all meetings. The Director-Finance will coordinate the submission of the 990 form by either mail or electronic means. The Director-Finance will retain proof of the mailing or electronic submission. A file will be established electronically and also a file for hard copies of the 990 filings.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Center for Human Services (CHS) has specific written policies for monitoring potential or actual conflicts of interest. Persons covered under the policy include any employee or volunteer (Board Members) with decision making authority. For volunteers (Board Members), the executive committee of the board determines whether a conflict exists. For employees, the executive director makes the final decision. Employees update their conflict of interest disclosure statement annually as do volunteers. Actual conflicts are reviewed by the Board of Directors, Executive Committee for Volunteers and by the executive director and members of the senior management team for employees. The Board executive committee reviews any actual or potential conflict involving the executive director. Certain restrictions may be applied, depending on the actual conflict for both volunteers and employees. Restrictions could include prohibiting participation in board deliberations and transactions and for employees prohibiting participation at identified meetings, etc.

Form 990, Part VI, Line 15A: Compensation Review & Approval Process - CEO, Top Management

Determining Compensation - The Executive Directors salary compensation is reviewed by members of the board who belong to either the executive, finance, or governance committees. A request for salary information from other local non profit agencies is sent out annually in survey format. A record of the deliberations and decision regarding the executive director compensation is contained in the board finance committee minutes. A similar process is used to determine salaries for key employees.

OTHER PROGRAM SERVICES 4: Other youth services include various activities and programs for youth (education, prevention and intervention). Specific programs include Friday Night Live and Life Plan. Center for Human Services operates with a strong youth development philosophy. Youth are our partners and all of our Youth Services programs serve to educate and strengthen young people as future leaders.

Financial Statement Notes

Part X : FIN48 Footnote

The Center for Human Services is exempt from Federal and California taxation on income under Section 501(c)(3) of the Internal Revenue Code and Section 23701(d) of the California Revenue and Taxation Code. Accordingly, no provision for income taxes has been included in the accompanying financial statements.Center for Human Services has accounted for uncertainty in income taxes as required by the Accounting for Uncertainty in Income Taxes topic of the FASB Accounting Standards Codification. Center for Human Services uses a comprehensive model for recognizing, and measuring, presenting and disclosing in the financial statements tax positions taken or expected to be taken on a tax return. A tax position is recognized as a benefit only if it is "more likely than not" that the tax position would be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized is the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the "more likely than not" test, no tax benefit is recorded. The effect of applying this model and the resulting identification of uncertain tax positions, if any, were not considered significant for financial reporting purposes.

IRS990/Desc0Behavioral Health programs provide mental health and substance abuse counseling services to community members in need. Individual, family and group counseling is provided at locations throughout the county. The First Step program provides a comprehensive continuum of substance abuse treatment services for pregnant and parenting women and their preschool age children (ages 0-5); the goal of the program is to assist women in maintaining drug and alcohol free lifestyles and to develop positive parenting skills. Clients learn to identify and implement healthy interpersonal relationships and to provide safe, sober, and nurturing home environments for themselves and their children; 100% of women participating in the First Step Program reported that their relationship with their child had improved. Student Assistance Program (SAP) provides short-term education, prevention and intervention services to students K-12 by building life, social and coping skills. Our top five contact reasons in this program relate to Anger Management, Academic, Anxiety, Family and Relationship Issues. During 2017-2018 the SAP program served 4,857 students. School staff are surveyed and consistently report that referred students benefit from SAP services.

IRS990/ProgSrvcAccomActy2Grp/Desc0Family Resource Centers (FRC) are a key prevention strategy for addressing many of the challenges facing families in rural Stanislaus communities. Services are conveniently located in Newman, Patterson, Ceres and Oakdale, California. Hundreds of individuals and families access a variety of services each month at each FRC location. Current FRC services include community outreach and engagement activities, strength based family assessments, case management and mental health counseling, parent education and skill building classes, assistance with translation and application for aid and insurance, transportation to appointments, and linkage to other resources as deemed appropriate. Additionally, individuals and families have access to other public/private services co-located at the sites. In addition to these services, over 1500 people received emergency food/crisis assistance through our Family Resource Centers.

IRS990/ProgSrvcAccomActy3Grp/Desc0Shelter services are provided through two programs: Hutton House, a temporary shelter for runaway, homeless and youth in crisis, ages 13-17, provides services in a residential setting for 8 youth at a time for a maximum of 15 days. In addition, day services are available for youth and their families and the crisis line is available 24 hours a day. During fiscal year 2017-2018, 234 youth received residential services at Hutton House and an additional 273 youth were served on a non-residential basis. 75% of all youth receiving services at Hutton House reported they had been taught skills to use when conflict happens at home. Pathways, a transitional living & supportive services program for 16 young adults, ages 18-22 years, serves homeless youth. In addition to housing, services include life skills and employment training, case management, mental health and substance abuse counseling and support in reaching educational goals. During fiscal year 2017-2018, Pathways served 33 residential participants and 84% of the young adults living at Pathways secured employment.
