“Form 990 is prepared by independent auditors. The finance and operations manager reviews the form 990 document and works with the outside accountants to complete the form. The final draft of the form 990 is reviewed by the executive director and the finance committee. The form 990 is then distributed to the entire board of directors prior to being submitted.”

“Once a year conflict of interest disclosure forms are distributed to the entire board using a confidential webform. Additionally, the disclosure form is part of the board manual and distributed at new member orientation. Once the disclosure forms are completed and submitted online, they are automatically sent to the operations director to review. If there are any conflicts of interest, they are discussed with the executive director and appropriate actions are taken.”

“The executive director's performance and compensation is reviewed annually by the executive committee of the board. They are provided with and review available comparability data received from an independent regional research/survey group when making their decision. The decision process is documented in the committee's meeting minutes.”

Since 1975, donate life northwest (dlnw) has aligned and supported collaborative partnerships with organ, eye, and tissue donation agencies, transplant centers, and educators. Recognized nationally for the achievement of registering more than 2.5 million of oregon's licensed drivers, program efforts focus on educating and motivating citizens of oregon and sw washington through our volunteers, high school and college outreach, social media, community organizations and department of motor vehicles to make, register, and share their donation decision.”

“Volunteer program: donate life northwest's volunteers are the heart of our mission, sharing their powerful personal stories to inspire others to register as organ, eye, and tissue donors. Their experiences-whether as recipients, donor families, living donors, or advocates-bring the message of donation to life in communities across oregon and southwest washington. Through outreach events, school presentations, and community partnerships, our trained volunteers build understanding, foster compassion, and motivate thousands to say "yes" to donation. In fy25, we had 150 active volunteers who gave 1071 hours of service.”
