“Form 990 is prepared by independent auditors. The finance and operations manager reviews the form 990 document and works with the outside accountants to complete the form. The final draft of the form 990 is reviewed by the executive director and the finance committee. The form 990 is then distributed to the entire board of directors prior to being submitted.”

“Once a year conflict of interest disclosure forms are distributed to the entire board using a confidential webform. Additionally, the disclosure form is part of the board manual and distributed at new member orientation. Once the disclosure forms are completed and submitted online, they are automatically sent to the operations director to review. If there are any conflicts of interest, they are discussed with the executive director and appropriate actions are taken.”

“The executive director's performance and compensation is reviewed annually by the executive committee of the board. They are provided with and review available comparability data received from an independent regional research/survey group when making their decision. The decision process is documented in the committee's meeting minutes.”

Dmv outreach: partnership with the department of motor vehicles provides oregonians the opportunity to register to be donors through permits, licenses and identification cards. Activities and resources include training to dmv regional and field office employees, collateral materials and targeted campaigns to individuals who have not registered to be an organ, eye and tissue donor.”
