Form 990, Part VI, Section B, line 12C

The CH Board of Directors shall take appropriate disciplinary action in the event that any director, officer or committee member fails to disclose a financial relationship that is an actual or possible conflict of interest or otherwise violates the terms and conditions of this policy. The Board's disciplinary action may include sanction, probation and removal from the Board or the Committee as the Board deems appropriate under the circumstances. The adoption and enforcement of this policy by the CH Board of Directors supplements, rather than replaces, the laws of the state of Washington for conflict of interest transactions by officers and directors of non-profit corporations.

Form 990, Part VI, Section B, line 15

CEO wages are reviewed annually by the board against performance outcomes and salary survey data from the United Way and local salary surveys. Staff wages are reviewed annually by the CEO against performance outcomes and salary survey data from the United Way and local salary surveys.

Part V, Line 4:

The agency's permanently restricted net assets are entirely compromised of one donor restricted endowment fund. The income from the fund can be used for general unrestricted purposes. The Agency's policy is to spend an appropriate investment income earned on the endowment in the year it is received.

Part X, Line 2:

Cocoon House is a Washington nonprofit corporation, exempt from federal income tax under Section 501(c)(3) of the Internal Revenue Code. It has been classified as an organization that is not a private foundation under Section 509(a) of the Internal Revenue Code. The Agency's income tax filings are subject to examination by various taxing authorities. The Agency follows the provisions of uncertain tax positions as addressed in FASB Accounting Standards Codification Subtopic 740-10, Income Taxes. The Agency believes that it has appropriate support for any tax positions taken, and as such, does not have any uncertain tax positions that are material to the financial statements.
