Prior to filing, senior management reviewed the 990 and provided a copy to all board members.

Form 990, Part VI, Section B, Line 12C

Annual completion of a "conflict of interest disclosure form" is required of all board members and staff each july. Forms are reviewed by the human resources manager and senior management team. Employees, officers and board members are required to avoid situations where real or potential conflicts of interest may arise. Career path services is determined to minimize or eliminate potential conflicts of interest that may arise from employees.

Form 990, Part VI, Section B, Line 15

Full salary survey is completed every three years and salary ranges approved by the board of directors. Discussion and decisions are documented in the board minutes. The board of directors conducts an annual performance review of the ceo and determines compensation.

Form 990, Part VI, Section C, Line 19

Governing documents, audit reports, and conflict of interest policy are available on our website or by request. Our 990's are available on various public websites or by request.
