Form 990, Page 6, Part VI, Line 12C

The organization has a conflict of interest policy. This policy requires annual, written disclosure of all potential conflicts of interest by board members and management on an annual basis. The executive committee reviews potential conflicts of interest. If a conflict of interest is determined to exist, the affected board member will be recused from voting on related issues. If the governing board or committee has reasonable cause to believe a member has failed to disclose actual or possible conflicts of interest, it shall inform the member of the basis for such belief and afford the member an opportunity to explain the alleged failure to disclose. If, after hearing the member's response and after making further investigation as warranted by the circumstances, the governing board or committee determines the member has failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective action.

Form 990, Page 6, Part VI, Line 15A

When salaries are set for employees, including the executive director, a review of comparative nonprofit salaries is made by consulting the utah nonprofits employee compensation survey printed annually. The determination and deliberation of salary for the executive director is determined independent of the executive director and is contemporaneously documented in the board minutes.

FORM 990 - ORGANIZATION'S MISSION

Using the arts and group dynamics to enhance youth development and build community. Form 990, Page 2, Part III, Line 4A

Expressive arts activities during 2016 included: -"transformations through face painting" -"creating with cyanotypes" -"self-exploration through mosaics" -"finding inspiration through collage" -"the art of storyboarding" -"mindfulness and meditation through yoga and music" -"exploring feelings with shoe shrines" -"collaborating together with murals" -"exploring sound through singing and voice?over" -"nature and art-sculpting wire insects" -"the art of science? Bubble art" -"reflections with self-portraits" -"finding inner peace through art journaling" -"de?stressing with watercolors". The youth in our programs responded positively to the various expressive art activities. More than 95 % of our youth felt the program was a positive experience for them and look forward to attending again. More than 89 % of our youth felt more confident in trying new artistic activities after attending the arts-kids program. 96 % of parents felt the arts-kids after-school program was a benefit to their children and would recommend the arts-kids program to other families. In 2016, arts-kids collaborated with an hbo film production. Some of our participants created murals as part of the set design. This opportunity gave our youth the chance to learn the importance of collaboration and practice creative problem?solving. Many youth showed great leadership and organizational skills and worked together to complete their art pieces on time. The youth who participated were very proud of the work they created and felt fortunate to be involved in such a unique and creative project. In 2016, arts-kids also had a disney/pixar animation artist help students understand the "art of storyboarding". Our youth were inspired by the fact the artist had worked on several video games and films they were familiar with. It was our most popular artists q & a session. Many youth chose to depict life events in their storyboards for this activity and it gave students a chance to share something new with their peers. In 2016, arts-kids programming connected arts and sciences through creative activities. For example, our youth gained self?confidence through the exploration and appreciation of new artistic formats, such as in a "bubble art" activity, while simultaneously learning science concepts like why bubbles are multi-colored. In a non-competitive and non-graded environment, our participants gained life skills, increased their appreciation for the arts, and realized the benefits of building a safe and supportive community for everyone in the arts-kids after-school program.

GRANTS AND PRIVATE DONATIONS MAKE IT POSSIBLE TO OFFER THE PROGRAM FREE TO ALL PARTICIPANTS REGARDLESS OF A FAMILYS ABILITY TO PAY.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt1EXPRESSIVE ARTS ACTIVITIES DURING 2016 INCLUDED: -"TRANSFORMATIONS THROUGH FACE PAINTING" -"CREATING WITH CYANOTYPES" -"SELF-EXPLORATION THROUGH MOSAICS" -"FINDING INSPIRATION THROUGH COLLAGE" -"THE ART OF STORYBOARDING" -"MINDFULNESS AND MEDITATION THROUGH YOGA AND MUSIC" -"EXPLORING FEELINGS WITH SHOE SHRINES" -"COLLABORATING TOGETHER WITH MURALS" -"EXPLORING SOUND THROUGH SINGING AND VOICE?OVER" -"NATURE AND ART-SCULPTING WIRE INSECTS" -"THE ART OF SCIENCE?
