This is documented within the minutes to the meeting. If an individual covered by the policy fails to disclose a potential conflict of interest, the governing board or committee will information the interested person of such belief. The individual is allowed the opportunity to explain the alleged failure to disclose. If, after hearing the interested person's response, and investigating further if warranted, the governing board or committee determines there was a failure to disclose, the governing board or committee will take appropriate disciplinary and corrective action.”
