Form 990, Page 2, Part III, Line 4A

The organization's vision is to empowering elders to gracefully live their next chapters. Operational plans are still being put in place, but three programs are anticipated: 1) dementia crisis care - sequel plans to operate a dementia resource facility in rapid city which will be staffed with qualified workers. Currently in our community, there are no centralized resources for families navigating dementia, and sequel aims to answer this cry for help and create centralized resources and a landing pad for those experiencing dementia and their families. This also offers the caregiver a much-needed respite. 2) elder angel funds - sequel will set up an elder angel fund which, through generous donations, helps to bridge the gap for vulnerable seniors. At risk seniors who live on a tightly fixed income are often not able to come up with any extra money for items like necessary medication co-pays, durable medical equipment, food, household goods or transportation. Sequel has an application approval process for at risk seniors to access a specified grant amount for a specific need. We believe that seniors should not have to choose between food and medication or between food and dressing supplies, but sadly this is a scenario that plays out frequently in our community. Sequel aims to change that. 3) case management - sequel will offer case management services to families navigating dementia in rapid city. It is our belief that no one should go through dementia alone and every senior deserves someone to stand by them through the disease process. Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.

OPERATIONAL PLANS ARE STILL BEING PUT IN PLACE, BUT THREE PROGRAMS ARE ANTICIPATED: 1) DEMENTIA CRISIS CARE - SEQUEL PLANS TO OPERATE A DEMENTIA RESOURCE FACILITY IN RAPID CITY WHICH WILL BE STAFFED WITH QUALIFIED WORKERS. CURRENTLY IN OUR COMMUNITY, THERE ARE NO CENTRALIZED RESOURCES FOR FAMILIES NAVIGATING DEMENTIA, AND SEQUEL AIMS TO ANSWER THIS CRY FOR HELP AND CREATE CENTRALIZED RESOURCES AND A LANDING PAD FOR THOSE EXPERIENCING DEMENTIA AND THEIR FAMILIES. THIS ALSO OFFERS THE CAREGIVER A MUCH-NEEDED RESPITE. 2) ELDER ANGEL FUNDS - SEQUEL WILL SET UP AN ELDER ANGEL FUND WHICH, THROUGH GENEROUS DONATIONS, HELPS TO BRIDGE THE GAP FOR VULNERABLE SENIORS. AT RISK SENIORS WHO LIVE ON A TIGHTLY FIXED INCOME ARE OFTEN NOT ABLE TO COME UP WITH ANY EXTRA MONEY FOR ITEMS LIKE NECESSARY MEDICATION CO-PAYS, DURABLE MEDICAL EQUIPMENT, FOOD, HOUSEHOLD GOODS OR TRANSPORTATION. SEQUEL HAS AN APPLICATION APPROVAL PROCESS FOR AT RISK SENIORS TO ACCESS A SPECIFIED GRANT AMOUNT FOR A SPECIFIC NEED. WE BELIEVE THAT SENIORS SHOULD NOT HAVE TO CHOOSE BETWEEN FOOD AND MEDICATION OR BETWEEN FOOD AND DRESSING SUPPLIES, BUT SADLY THIS IS A SCENARIO THAT PLAYS OUT FREQUENTLY IN OUR COMMUNITY. SEQUEL AIMS TO CHANGE THAT. 3) CASE MANAGEMENT - SEQUEL WILL OFFER CASE MANAGEMENT SERVICES TO FAMILIES NAVIGATING DEMENTIA IN RAPID CITY. IT IS OUR BELIEF THAT NO ONE SHOULD GO THROUGH DEMENTIA ALONE AND EVERY SENIOR DESERVES SOMEONE TO STAND BY THEM THROUGH THE DISEASE PROCESS. CASE MANAGEMENT IS A COLLABORATIVE PROCESS OF ASSESSMENT, PLANNING, FACILITATION, CARE COORDINATION, EVALUATION AND ADVOCACY FOR OPTIONS AND SERVICES TO MEET AN INDIVIDUAL'S AND FAMILY'S COMPREHENSIVE HEALTH NEEDS THROUGH COMMUNICATION AND AVAILABLE RESOURCES TO PROMOTE PATIENT SAFETY, QUALITY OF CARE, AND COST-EFFECTIVE OUTCOMES.
