Form 990, Part IX, Line 11G

Other: program service expenses 218,658. Management and general expenses 0. Fundraising expenses 3,900. Total expenses 222,558.

FORM 990, PART XII, LINE 2C:

The organization's finance committee assumes responsibility for the oversight of the audit. The finance committee was established in fiscal year 2013 and consists of the treasurer, a cpa board member, the fiscal agent, and acting executive director.

Schedule A, Part II, Line 1

Schedule a has been adjusted to reflect the analysis of government contacts. Under regulation 1.509(a)-3 a portion of the government contracts should be considered grants/contributions because the payments were made to allow the organization to provide a direct benefit to the general public. The revenue related to peer counselors has been reclassifed from program service revenue to grant revenue for all years.

Financial Statement Notes

PART X, LINE 2:

The task force applies the provisions of financial accounting standards board (fasb) accounting standards codification (asc) 740, accounting for income taxes. There were no uncertain tax positions taken by the task force for the years ended september 30, 2016 or 2015. The task force's policy is to classify income tax penalties and interest, when applicable, according to their natural classification.
