The organization's "founding members" are el paso county public health, uchealth memorial health system, peak vista community health centers, penrose-st. Francis health services, el paso county medical society, and diversus health. Each of the founding members has the right to appoint one director so long as such entity's organizational missions continue to support the corporation's mission, vision, and purpose. Additionally, if a founding member desires to replace its appointed director, it may do so provided the replacement director has the willingness and ability to support the corporation's mission, vision, and purpose and meets the criteria set forth by the chp board of directors.

As a result of its work, it recently launched the prism provider directory, which is the first of its kind in the pikes peak region. This free, searchable database allows lgbtq+ individuals to find inclusive, affirming health care professionals.
