FORM 990, PART III, LINE 1:

In 2019, CHP adopted Collective Impact as its primary collaboration framework and we act as a backbone entity for four complex systems change initiatives: suicide prevention, homelessness, substance use disorder (with an emphasis on opioid use disorder), and LGBTQ+ health equity. We convene hundreds of organizations and individuals to guide vision and strategy, advance public policy, build public will, maintain continuous communication, support aligned activities, establish shared measurement practices, and mobilize funding. CHP exists to drive continuous improvement in the health and well-being of our community. Form 990, Part III, Line 3: Discontinuation of CATCH program. CATCH services provided to patients consist of volunteer services offered to CHP patients for special health care services (i.e. cardiology). Program is being eliminated as a CHP program. FORM 990, PART III, LINE 4B: CHP RECRUITS PHYSICIANS, HOSPITALS, MEDICAL SERVICES AGENCIES, SAFETY NET CLINICS, AND VOLUNTEER MEDICAL PROVIDERS TO DONATE THEIR EXPERTISE AND CLINICAL RESOURCES. CATCH IS MUTUALLY BENEFICIAL TO THE COMMUNITY BECAUSE IT CONNECTS LOW-INCOME RESIDENTS TO DONATED HEALTH CARE SERVICES WHILE ENABLING MEDICAL PROVIDERS WHO WANT TO VOLUNTEER TO DO SO IN A COORDINATED, EFFICIENT MANNER. AMONG ITS ACCOMPLISHMENTS IN THE PAST YEAR, THE CATCH PROGRAM HAS MADE REFERRALS TO PRIMARY AND SPECIALTY CARE IN DONATED PHYSICIAN SERVICES, AND PROVIDED IN DONATED X-RAYS, LABORATORY SERVICES, AND SPECIALTY TESTS. THESE SERVICES WERE VALUED AT $224,570 DURING JUNE 30, 2021. SINCE THE PROGRAM'S INCEPTION IN 2006, CATCH HAS FILLED MORE THAN 103,000 PRESCRIPTIONS EQUALING MORE THAN $17.3 MILLION. Amount consists of CATCH services provided to patients. PROGRAM WAS ELIMINATED IN FYE 2021. FORM 990, PART III, LINE 4C: As the lead agency, CHP is the collaborative applicant for the PPCoC and helps lead the local strategic vision, prepares applications for federal funding, convenes community partners, manages data for the Homeless Management Information System and Coordinated Entry Program, and advocates for policies and best practices. FORM 990, PART III, LINE 4D: CHP supports the Coalition for Prevention Education, Addiction and Recovery (CPAR). CPAR is a collective impact organization dedicated to building the community's capacity to respond to substance use disorder (SUD) and helping to establish an effective and accessible continuum of behavioral health care. CPAR partners with a wide and growing cross-section of government organizations, nonprofits, and private entities that are involved in prevention, treatment, recovery, crisis management, criminal justice, emergency response, peer navigation, and other related fields. The partnership works to pool collective knowledge and resources to identify and resolve systemic obstacles to resource access for individuals experiencing SUD and their families. FORM 990, PART VI, SECTION A, LINE 6: CLASSES OF MEMBERS OR STOCKHOLDERS: THE MEMBERS OF COMMUNITY HEALTH PARTNERSHIP ARE PRIMARILY ORGANIZATIONS WITH A MISSION TO SERVE PEOPLE WITHIN THE CORPORATION'S SERVICE AREA, OTHER ORGANIZATIONS WHICH DELIVER HEALTH SERVICES TO PEOPLE IN THE SERVICE AREAS, OR INDIVIDUALS WHO HAVE AN INTEREST IN IMPROVING THE OVERALL SYSTEM OF HEALTH CARE.
