Governance Explanations

Form 990, Page 6, Part VI, Line 11B

A copy of the 990 is provided to the executive director and the board treasurer who review the form, schedules and related attachments. Any comments or questions are addressed with the preparer. Once management is satisfied with the 990, the executive director signs the form 8879-eo authorizing the preparer to e-file the return.

Form 990, Page 6, Part VI, Line 12C

All directors and officers of the board complete a conflict of interest questionnaire annually. The questionnaires are reviewed by the board president and directors will disclose or work to resolve any actual or potential conflicts. In the absence of resolution, the board president will refer the matter to the board.

Form 990, Page 6, Part VI, Line 15A

The finance committee reviews and determines the executive director's compensation during succession planning using comparable data. The committee performed a review of ceo compensation levels in the pacific northwest region as well as other national organizations conducting similar work; vrlc, rainn, iafn, etc. The executive director's compensation was well within this range and considered reasonable.

Form 990, Page 6, Part VI, Line 15B

The executive director is the only officer or key employee. See 990, part vi, line 15a.

Form 990, Page 2, Part III, Line 4D

The start by believing campaign (sbb) is dedicated to changing the way society responds to victims of sexual assault to make it easier and safer to seek help. Evawi had numerous agencies launch their own start by believing campaign. In 2014, arizona became a start by believing state. Also, sprint has become a supporter, donating to evawi per line activation in california and arizona.
