Form 990, Page 6, Part VI, Line 11B

A copy of the 990 is provided to the ceo and the board treasurer who review the form, schedules and related attachments. Any comments or questions are addressed with the preparer. Once the ceo and the treasurer approve the 990, the ceo signs form 8879-eo authorizing the preparer to efile the return and the 990 is provided to the full board of directors.

Form 990, Page 6, Part VI, Line 12C

All directors and officers complete a conflict or duality of interest disclosure annually. Our board of directors and staff conflict or duality of interest policy and the disclosure form were revised and approved by the board of directors on january 16, 2019. Directors completed their disclosures in january of 2019 and the forms were reviewed by the board president and the ceo. If applicable, the board president and ceo work to resolve any actual or potential conflicts. In the absence of resolution, the board president would refer the matter to the executive committee and depending on the outcome, the full board of directors. There were no unresolved conflicts of interest identified in 2019 or 2020 to date.

Form 990, Page 6, Part VI, Line 15A

The financial review committee establishes the compensation of our chief executive officer (ceo). To establish executive compensation, the financial committee uses comparable data generated by charity navigator's formula for non-profit organizations. Charity navigator is a well-known research organization that surveys non-profit organizations across the country and creates a standard for the industry. The formula for ceo compensation is based on several factors, including geographic region, size and mission of the organization, as well as annual revenues and performance. Evawi is considered a mid-to-large sized charity, which is defined as a charity with 1 million or more in revenues. When a change in executive compensation is under consideration, the financial committee makes a recommendation to the board of directors based on the charity navigator formula and other factors such as performance. The board of directors votes to approve or reject the recommendation. The minutes for the board meeting are filed in accordance with evawi's bylaws, documenting deliberations and the basis for a decision. Although we had been following these procedures since our ceo and founder first received compensation, policy no. 27, executive compensation was approved by the board of directors on february 19, 2020 to ensure the procedures are documented in policy.

Form 990, Page 6, Part VI, Line 15B

The ceo is the only paid officer or key employee. See 990, part vi, line 15a.

Form 990, Page 6, Part VI, Line 19

In 2019, two new financial policies were written, 7 new personnel policies were added to the manual and 11 policies were updated. All new and updated policies were approved by the board of directors. In addition, the financial review committee reviews our entire financial policy manuals every three years. The committee last reviewed all 51 financial policies in 2018. In 2019, the executive committee reviewed all 79 policies to ensure that they remain up-to-date and to confirm that board and staff members are complying with evawi policies and procedures. To promote transparency, many of evawi's policies can be found on our website under about us and disclosures (code of business conduct and ethics, discrimination and harassment, whistleblower reporting and response, board of directors and staff conflict or duality of interest).
