Governance Explanations

Form 990, Part VI, Section B, line 11

Leap engages a third party accounting firm to prepare the form 990. Leap's director of finance is the staff liaison to the accounting firm and upon the initial completion of form 990, the director of finance reviews and edits the return. After management approval, the return is submitted to the board for review and final approval. After all approvals are confirmed, form 990 is filed with the irs.

Form 990, Part VI, Section B, line 12C

Employees and board members are required to complete or update a conflict of interest statement on an annual basis. On these statements, employees and board members must disclose any interests in a proposed transaction or decision that may create a conflict of interest. After disclosure, the employee or board member will not be permitted to participate in the transaction or decision. These statements are typically completed and signed at the first board meeting of the year.

Financial Statement Notes

Part X, Line 2:

Management has concluded that any tax positions that would not meet the more-likely-than-not criterion of financial accounting standards board (fasb) accounting standards codification (asc) topic 740-10, accounting for income taxes, would be immaterial to the financial statements taken as a whole. Accordingly, the accompanying financial statements do not include any provision for uncertain tax positions, and no related interest or penalties have been recorded in the operating statements or accrued in the statements of financial position. Federal and state tax returns for leap are generally open to examination by the relevant taxing authorities for a period of three years from the date the returns are filed.

IRS990/Desc0Mission Program: LEAP conducts several surgical mission trips each year. The LEAP team is comprised of approximately 275 medical volunteers, such as physicians, nurses and other medical support staff and non-medical volunteers who dedicate their time, expertise and often their own financial resources. The people served by LEAP live in regions with limited medical resources and almost no access to specialized surgical care. The current outreach includes annual mission trips to Belize, Dominican Republic, India, Zimbabwe and bi-annual mission trips to Haiti. In 2015, LEAP performed over 300 life-changing, highly specialized surgeries to those in need. LEAP strives to further education and expertise in all countries in which it operates. LEAP also provides support and education to patients' families in order to help them welcome their loved ones back into the community as participating members. Through this approach, LEAP is able to equip and empower those served for a lasting impact

IRS990/ProgSrvcAccomActy2Grp/Desc0Landmark Program: LEAP, in partnership with Medical City Children's Hospital in Dallas, makes it possible for children with the most complex surgical needs, who live in underserved areas of the world, to travel to the United States to receive high quality medical and surgical care. Many of the children served by LEAP have such severe craniofacial abnormalities that they have been abandoned as infants or sent to palliative care facilities. LEAP often takes care of or greatly reduces their travel and surgical costs and also coordinates host families who will offer these children a loving and safe environment while they heal. Through this program, LEAP has performed reconstructive surgery on children from the Bahamas, China, Russia, Dominican Republic, Mexico, Haiti, Middle East and Honduras. To date, approximately 50 children have had their lives radically transformed because of the LEAP Landmark Program. In 2015, LEAP served three children through the Landmark Program by performing life-saving surgeries valued at approximately $862,000. Approximately 95% of these services were performed free of charge by physicals and hospitals dedicated to these unique LEAP patients.

IRS990/ProgSrvcAccomActy3Grp/Desc0International Disaster Relief Program: LEAP responds to international disasters by partnering with the International Society of Aesthetic Plastic Surgery (ISAPS) to create a global resource for surgical teams to respond quickly to worldwide disasters in order to save life and limb through the ISAPS-LEAP Surgical Relief Teams effort. This partnership began in October 2013 when the first ISAPS-LEAP Surgical Relief Teams mission was sent to Jordan to serve wounded Syrian refugees. In 2014, the ISAPS-LEAP Surgical Relief Teams effort led 16 self-funded mission teams to Jordan and Lebanon to serve over 230 Syrian refugees. In 2015, five ISAPS-LEAP Surgical Relief Teams traveled to Turkey and Jordan and served approximately 114 people in need.
