Supplemental Narrative

Additional Explanations

Pt VI, Line 11B

Form 990 is reviewed by the finance committee. Copies are distributed via email to all board members before filing.

Pt VI, Line 12C

Members of the board of directors and employees annually review and sign a conflict of interest policy. All employees review and sign a conflict of interest policy at hiring. Annually board members, employees, and volunteers must disclose to the executive director/board chair, in written form, any business or non-business personal relationships deemed to create a conflict of interest. Any such persons will abstain from deliberations or voting that relates to the disclosed relationship.

Pt VI, Line 15A

The executive committee of the board of directors determines the executive director's annual compensation. Demographic compensation and benefit comparisons are obtained.

Pt VI, Line 15B

Annual compensation for all employees, other than the executive director, is recommended by the executive director for approval by the finance committee. After review, the finance committee makes its recommendation to the board of directors in the annual operating budget, which includes compensation, and is voted on by the board.

Pt VI, Line 19

Cancer care service's governing documents, financial statements, and conflict of interest policy are available upon request or on the organization's website.

Pt XI

The unrealized gains on investments held are realized for book purposes but not for tax purposes.

Pt XII, Line 2C

There is an audit committee that oversees the annual audit process, and the selection of an outside auditor. There has been no change to that process.

Pt VI, Line 2

Two of the board members work at the same firm. One is a partner and the other two are employees.

Financial Statement Notes

Pt V, Line 4

THE BOARD OF DIRECTORS OF CANCER CARE SERVICES REQUIRES THE PRESERVATION OF THE FAIR VALUE OF THE ORIGINAL ENDOWMENT GIFT OF THE GIFT DATE OF THE DONOR-RESTRICTED ENDOWMENT FUNDS, ABSENT EXPLICIT DONOR STIPULATIONS TO THE CONTRARY. AS A RESULT OF THIS INTERPRETATION, CANCER CARE SERVICES CLASSIFIES AS PERMANENTLY RESTRICTED NET ASSETS: (a) THE ORIGINAL VALUE OF GIFTS DONATED TO THE PERMANENT ENDOWMENT, (b) THE ORIGINAL VALUE OF SUBSEQUENT GIFTS TO THE PERMANENT ENDOWMENT, AND (c) ACCUMULATIONS TO THE PERMANENT ENDOWMENT MADE IN ACCORDANCE WITH THE DIRECTION OF THAT APPLICABLE DONOR GIFT INSTRUMENT AT THE TIME THE ACCUMULATION IS ADDED TO THE FUND. THE REMAINING PORTION OF THE DONOR-RESTRICTED ENDOWMENT FUND THAT IS NOT CLASSIFIED IN PERMANENTLY RESTRICTED NET ASSETS IS CLASSIFIED AS TEMPORARILY RESTRICTED NET ASSETS UNTIL THOSE AMOUNTS ARE APPROPRIATED FOR EXPENDITURE BY THE ORGANIZATION IN A MANNER CONSISTENT WITH THE STANDARD OF PRUDENCE PRESCRIBED BY UPMIFA.DISTRIBUTIONS FROM ENDOWMENT FUNDS ARE TO SUPPORT PROGRAMS AND THE OPERATIONS OF CANCER CARE SERVICES.
