LITTLE SAMARITAN MONTESSORI CENTER MIDTOWN PUBLIC CHARTER SCHOOLSENIOR CASE MANAGEMENT (RE-ESTABLISHED IN 2008): CASE MANAGEMENT PROGRAM FOCUSES ON SOCIAL SERVICES AND CASE MANAGEMENT FOR HOMEBOUND SENIORS AND DISABLED INDIVIDUALS LIVING IN THE MIDTOWN NEIGHBORHOOD.
