THE PROGRAM BEGINS WITH DELIVERY OF A NOON MEAL, AND CONTINUES WITH ROUTINE HOME VISITS FROM COMMUNITY OUTREACH STAFF. THE DIRECTOR OF COMMUNITY OUTREACH DETERMINES ELIGIBILITY BASED ON AN ASSESSMENT.
