Form 990, Part VI, Section B, Line 11B

Form 990, Part VI, Section B, Line 12C

The organization monitors and enforces compliance with the conflict of interest policy by annually requesting this information from the board members and reviewing it with all new board members. All employees must disclose outside employment or engagement in consulting for others in writing, prior to the commencement of such outside employment or consulting to human resources, unless it is beyond dispute that such activity could not present a conflict of interest, potential conflict of interest, or the appearance of a conflict of interest. All conflicts of interest must be resolved. Authorization to engage in conduct that constitutes a potential conflict of interest or the appearance of a conflict of interest must be obtained from the executive director. In order to avoid conflicts of interest, chrysalis house does not employ immediate family members of any current employee. Violations of this policy may result in discipline, including the possibility of immediate termination.

Form 990, Part VI, Section B, Line 15

The chrysalis house executive committee determined compensation for the executive director and human resources determined compensation for key employees through review of comparability data on o*net online, online 990 reports, and telephonic calls with regional substance abuse treatment programs. The compensation is approved by the board of directors as part of the annual budget.

Form 990, Part VI, Section C, Line 19

The organization does not make its governing documents, financial statements and conflict of interest policy available to the public.
