Activity 2

The objecive of healthy families and relationships impact area is to provide families with the support they need to tackle life's challenges and transitions, through parenting skills training, counseling on mental health, support groups dealing with family issues, and school programs that prepare children for success and enhance parent-child communications. This impact area served 15,846 clients during the year. The counseling program resulted in 92% of the clients served made progress toward treatment goals. The outcomes from the healthy babies healthy moms program showed 93% of the babies born weighed 5.5 pounds or more, 91% of the babies born were full- term births, 91% of the clients were actively using contraception to delay a subsequent pregnancy, and 83% of the teens returned to school within six weeks of the baby's birth or received educational support services. The outcomes of the chispa program showed 73% of parents became more engaged and increased their knowledge, 91% of parents participated in their child's transition plan to school, 94% of parents participated in community activities, 100% of parents participated in school activities, and 100% of the children were developmentally on track.

Activity 3

The objective of family sustainability and empowerment impact area is to provide housing-based programs promoting healthy environment, self- sufficiency, child safety, and battle poverty, homelessness, mental illness and substance abuse. The impact area served 3,254 clients this year. The outcomes of the weaver gardens program showed 90% of residents maintained housing stability for 90 days or more, 100% of residents were employed or in an educational/vocational program, and 100% of babies have maintained current immunizations. The outcomes of the shelter a family program showed 94% of clients received regular mental health services, 100% of clients received regular physical health services, 100% of clients had regular group attendance to help prevent a relapse, 92% of clients' children received health and dental screenings annually, and 97% of clients' children were promoted to the next grade level.
