FORM 990 - ORGANIZATION'S MISSION

Provide a community-based alternative to prevent or delay the institutionalization of children and adults with disabilities, and to assist older adults to remain in their own homes, with family or primary caregivers.

Form 990

Trinity in-home care (tihc) became a 501(c)(3) in 1976. However, services began as a volunteer outreach of trinity episcopal church in the late 1960s. Over the 40+ years, tihc has evolved from training foster families to care for children with disabilities and serving only 5 families with in-home support services for children with developmental disabilities, to now serving over 300 individuals per year-as young as five years old through the end of life, and with a wide range of disabilities. No longer affiliated with trinity episcopal church, tihc services now focus on offering the most affordable, highest quality in-home and community based supports and respite care for individuals across douglas county. Tihc's clients are elderly or have a physical or intellectual disability which-- if it weren't for the services and care received from tihc's many caring direct support professionals (caregivers) -- could negatively impact their ability to live in the home where they are most happy and healthy, preventing or delaying a nursing facility or institutionalization. An additional benefit is this in turn saves tax-payer dollars, as it extends the opportunity for this lower-expense, more personal care for those who have their care paid for by state or federal funds. Private donors to tihc have the pleasure of knowing their gift is focused on client services. Tihc is able cover all overhead out of earned revenue, while focusing donations towards supplementing services to clients who qualify for medicaid (aka kancare, an underfunded program in kansas) home and community based services (hcbs) or who qualify for tihc's sliding scale, designed to assist low-income elderly who could not afford the cost of services on their own. Donations also subsidize tihc when a client meets catastrophic circumstances and are unable to pay their out-of-pocket expenses. This also allows tihc to budget subsidized services over the course of the year, meaning a sliding scale client's care never "runs out of funds" part way through the year. The core of trinity in-home care's mission is connecting caregivers to clients. This mission looks different than many other nonprofit agencies because, due to department of labor regulations, we must use employees-not volunteers-to carry it out. It is a complex process, as each of our 300 individual clients' specific needs change as their health changes, as does the time of day they need help. The crux of this is the administrative process of coordinating services which facilitates the critical trusting relationship between our clients and our employees. Tihc differs from other area medicaid hcbs providers because we recruit, hire, supervise and schedule caregivers for our clients, most of whom would be unable to hire on their own or who do not have family available to assist them. This is referred to in the industry as "agency directed services". Tihc works directly with other local agencies to prevent duplication of services and correctly refer clients to the agency most appropriate for their care. However, tihc is currently the only agency in douglas county providing this specific scope of services, depth of reduced rates and personalized options for supportive care. When tihc conducts annual surveys, we routinely receive feedback that expresses the magnitude of what tihc's employees mean to our clients. They reiterate year after year that their "quality of life could not be achieved without their tihc support", that the family members of clients are "relieved to know their loved one is safe so they can go to work", and that clients "don't know what they'd do without tihc" to help them sustain their independence in their home. New clients tell us that word-of-mouth is the most frequent referral source, which reflects most of all how much our clients trust and value trinity in-home care. The organization is exempt from income taxes under section 501(c)(3) of the internal revenu code as provided by a determination letter received from the internal revenue service. The organization adopted the provisions of fasb asc 740-10, accounting for income taxes. This standard clarifies the accounting for uncertainty in income taxes recognized in an organization's financial statements and prescribes recognition and measurement of tax positions taken or expected to be taken on a tax return that are not certain to be realized. The organization has not identified any uncertain tax positions for the year ended december 31, 2017. The organization's income tax returns are subject to review and examination by u.s. Federal jurisdiction and the state of kansas. The organization is not aware of any activities that are subjet to tax on unrelated business income or excise or other taxes.

HOWEVER, SERVICES BEGAN AS A VOLUNTEER OUTREACH OF TRINITY EPISCOPAL CHURCH IN THE LATE 1960S. OVER THE 40+ YEARS, TIHC HAS EVOLVED FROM TRAINING FOSTER FAMILIES TO CARE FOR CHILDREN WITH DISABILITIES AND SERVING ONLY 5 FAMILIES WITH IN-HOME SUPPORT SERVICES FOR CHILDREN WITH DEVELOPMENTAL DISABILITIES, TO NOW SERVING OVER 300 INDIVIDUALS PER YEAR-AS YOUNG AS FIVE YEARS OLD THROUGH THE END OF LIFE, AND WITH A WIDE RANGE OF DISABILITIES. NO LONGER AFFILIATED WITH TRINITY EPISCOPAL CHURCH, TIHC SERVICES NOW FOCUS ON OFFERING THE MOST AFFORDABLE, HIGHEST QUALITY IN-HOME AND COMMUNITY BASED SUPPORTS AND RESPITE CARE FOR INDIVIDUALS ACROSS DOUGLAS COUNTY. TIHC'S CLIENTS ARE ELDERLY OR HAVE A PHYSICAL OR INTELLECTUAL DISABILITY WHICH-- IF IT WEREN'T FOR THE SERVICES AND CARE RECEIVED FROM TIHC'S MANY CARING DIRECT SUPPORT PROFESSIONALS (CAREGIVERS) -- COULD NEGATIVELY IMPACT THEIR ABILITY TO LIVE IN THE HOME WHERE THEY ARE MOST HAPPY AND HEALTHY, PREVENTING OR DELAYING A NURSING FACILITY OR INSTITUTIONALIZATION. AN ADDITIONAL BENEFIT IS THIS IN TURN SAVES TAX-PAYER DOLLARS, AS IT EXTENDS THE OPPORTUNITY FOR THIS LOWER-EXPENSE, MORE PERSONAL CARE FOR THOSE WHO HAVE THEIR CARE PAID FOR BY STATE OR FEDERAL FUNDS. PRIVATE DONORS TO TIHC HAVE THE PLEASURE OF KNOWING THEIR GIFT IS FOCUSED ON CLIENT SERVICES. TIHC IS ABLE COVER ALL OVERHEAD OUT OF EARNED REVENUE, WHILE FOCUSING DONATIONS TOWARDS SUPPLEMENTING SERVICES TO CLIENTS WHO QUALIFY FOR MEDICAID (AKA KANCARE, AN UNDERFUNDED PROGRAM IN KANSAS) HOME AND COMMUNITY BASED SERVICES (HCBS) OR WHO QUALIFY FOR TIHC'S SLIDING SCALE, DESIGNED TO ASSIST LOW-INCOME ELDERLY WHO COULD NOT AFFORD THE COST OF SERVICES ON THEIR OWN. DONATIONS ALSO SUBSIDIZE TIHC WHEN A CLIENT MEETS CATASTROPHIC CIRCUMSTANCES AND ARE UNABLE TO PAY THEIR OUT-OF-POCKET EXPENSES. THIS ALSO ALLOWS TIHC TO BUDGET SUBSIDIZED SERVICES OVER THE COURSE OF THE YEAR, MEANING A SLIDING SCALE CLIENT'S CARE NEVER "RUNS OUT OF FUNDS" PART WAY THROUGH THE YEAR. THE CORE OF TRINITY IN-HOME CARE'S MISSION IS CONNECTING CAREGIVERS TO CLIENTS. THIS MISSION LOOKS DIFFERENT THAN MANY OTHER NONPROFIT AGENCIES BECAUSE, DUE TO DEPARTMENT OF LABOR REGULATIONS, WE MUST USE EMPLOYEES-NOT VOLUNTEERS-TO CARRY IT OUT. IT IS A COMPLEX PROCESS, AS EACH OF OUR 300 INDIVIDUAL CLIENTS' SPECIFIC NEEDS CHANGE AS THEIR HEALTH CHANGES, AS DOES THE TIME OF DAY THEY NEED HELP. THE CRUX OF THIS IS THE ADMINISTRATIVE PROCESS OF COORDINATING SERVICES WHICH FACILITATES THE CRITICAL TRUSTING RELATIONSHIP BETWEEN OUR CLIENTS AND OUR EMPLOYEES. TIHC DIFFERS FROM OTHER AREA MEDICAID HCBS PROVIDERS BECAUSE WE RECRUIT, HIRE, SUPERVISE AND SCHEDULE CAREGIVERS FOR OUR CLIENTS, MOST OF WHOM WOULD BE UNABLE TO HIRE ON THEIR OWN OR WHO DO NOT HAVE FAMILY AVAILABLE TO ASSIST THEM. THIS IS REFERRED TO IN THE INDUSTRY AS "AGENCY DIRECTED SERVICES". TIHC WORKS DIRECTLY WITH OTHER LOCAL AGENCIES TO PREVENT DUPLICATION OF SERVICES AND CORRECTLY REFER CLIENTS TO THE AGENCY MOST APPROPRIATE FOR THEIR CARE. HOWEVER, TIHC IS CURRENTLY THE ONLY AGENCY IN DOUGLAS COUNTY PROVIDING THIS SPECIFIC SCOPE OF SERVICES, DEPTH OF REDUCED RATES AND PERSONALIZED OPTIONS FOR SUPPORTIVE CARE. WHEN TIHC CONDUCTS ANNUAL SURVEYS, WE ROUTINELY RECEIVE FEEDBACK THAT EXPRESSES THE MAGNITUDE OF WHAT TIHC'S EMPLOYEES MEAN TO OUR CLIENTS. THEY REITERATE YEAR AFTER YEAR THAT THEIR "QUALITY OF LIFE COULD NOT BE ACHIEVED WITHOUT THEIR TIHC SUPPORT", THAT THE FAMILY MEMBERS OF CLIENTS ARE "RELIEVED TO KNOW THEIR LOVED ONE IS SAFE SO THEY CAN GO TO WORK", AND THAT CLIENTS "DON'T KNOW WHAT THEY'D DO WITHOUT TIHC" TO HELP THEM SUSTAIN THEIR INDEPENDENCE IN THEIR HOME. NEW CLIENTS TELL US THAT WORD-OF-MOUTH IS THE MOST FREQUENT REFERRAL SOURCE, WHICH REFLECTS MOST OF ALL HOW MUCH OUR CLIENTS TRUST AND VALUE TRINITY IN-HOME CARE.
