Mission

Care Guide Partners, Inc. is a non-profit organization whose express purpose if to develop care models for individuals with chronic and advanced illnesses who need medical and psychosocial assistance in their homes, workplace, or other locations designated by the individual. Care Guide Partners, Inc. provides adult and pediatric palliative care in thirty-five counties in Kentucky and provides pediatric care in six counties in Indiana.

Governance Explanations

Form 990, Part VI, Section A, Line 1A

The executive committee shall be convened by the chairperson when a matter of business requires action during the interim between regular meeting of the board of directors, or in the event that a regular meeting of the board cannot take action due to the absence of a quorum at the regular meeting. The executive committee shall take no action in conflict with the express policies of the board. The executive committee shall evaluate the performance of the president/ceo at least annually and shall review and discuss the evaluation with the president/ceo. The executive committee present to the board of directors recommendations regarding compensation for the president/ceo.

Form 990, Part VI, Section B, Line 11B

The form 990 is reviewed by the organization's cfo and senior director of finance. A copy of the form 990 is also provided to the board of directors for review prior to filing.

Form 990, Part VI, Section B, Line 12C

The board of directors and officers are required to complete a conflict of interest questionnaire annually. The board and officers are questioned as to any new concerns or potential conflicts of interest and such concerns are discussed at quarterly board meetings. Questionnaires are reviewed and analyzed prior to the filing of the form 990. Board members are asked to recuse themselves from any discussion on topics where they have conflicts of interest.

The board of directors and officers are reponsible for the oversight of the audit and approval of the annual audited financial statements as well as the selection of the independent accounts.

Financial Statement Notes

PART X, LINE 2:

The organization is exempt from income taxes on income from related activities under section 501(c)(3) of the u.s. Internal revenue code and corresponding state tax law. Accordingly, no provision has been made for federal or state income taxes. U.s. Gaap requires that a tax position is recognized as a benefit only if it is "more likely than not" that the tax position would be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized is the largest amount of tax benefit that is greater than 50% likely being realized on examination. For tax positions not meeting the "more likely than not" test, no tax benefits is recorded. The organization does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The organization's irs form 990 has not been subject to examination by the internal revenue service or the commonwealth of kentucky for the last three years. The organization recognizes interest and/or penalties related to income tax matters in income tax expense. The organization did not have any amounts accrued for interest and penalties at december 31, 2024 and 2023. The organization's irs form 990 is subject to examination generally for three years after the filing date.

WE WORK TO HELP DEFINE PERSONAL GOALS AND NEEDS TO CREATE AN INDIVIDUALIZED CARE PLAN TO HELP IMPROVE THE QUALITY OF LIFE FOR THE PATIENT AND THEIR FAMILY, AND TO HELP THEM TO UNDERSTAND THEIR TREATMENTS AND CHOICES. THE PALLIATIVE CARE TEAMS MAKE RECOMMENDATIONS TO LESSEN THE EFFECTS OF SYMPTOMS SUCH AS PAIN, SHORTNESS OF BREATHE, FATIGUE, CONSTIPATION, NAUESEA, LOSS OF APPETITE, AND DIFFICULTY SLEEPING. PALLIATIVE CARE MAY INCLUDE SPECIALTY VISITS BY A NURSE PRACTITIONER OR PHYSICIAN IN ADDITION TO OTHER INTERDISCIPLINARY TEAM SPECIALISTS LIKE SOCIAL WORKERS, NURSES, CNA'S, CHAPLAINS AND GRIEF COUNSELORS. PATIENTS RECEIVING PALLIATIVE CARE DO NOT HAVE TO DISCONTINUE CURATIVE TREATMENT FOR THEIR ILLNESSES. IN 2023, WE SERVED 2,519 PATIENTS IN OUR ADULT PALLIATIVE CARE PROGRAM.
