The purpose of path is to support the louisiana public health institute through the collaborative efforts with the institute in health information, research, educational and training opportunities, and to promote positive health practices through collaboration in program initiatives and policy development, operating a health information exchange, and other activities.

Form 990, Part VI, Section B, Line 11

The review of form 990 is scheduled at a quarterly board meeting. The independent board will review and approve form 990 before it is submitted to the internal revenue service. If the form 990 is due before the next scheduled board meeting, then a meeting is scheduled with the audit and finance committee. This committee will review and approve the form 990 prior to signatures and submission.

Form 990, Part VI, Section B, Line 12C

To ensure that the organization operates in a manner consistent with charitable purposes, the organization performs periodic reviews of the conflict of interest policy. Board members sign a conflict of interest form annually. The ceo and director of finance and operations reviews this to ensure no areas of concern. In addition, any conflicts of interest are discussed annually in the audit report and disclosed as applicable.

Form 990, Part VI, Section B, Line 15

Path has no employeess. Management, personnel, and support services were provided to path by lphi (ein:72-1379921), path's supported organization. Lphi has a compensation committee which reviews the compensation information. Lphi evaluates each job within the organization and analyzes the relative worth compared to other jobs within the organization. The resulting classification of the job determines its relative pay position in the organization's hierarchy. Job evaluation compares jobs to ensure that positions within similar levels of complexity and responsibility, requiring similar levels of experience, are classified in the same grade in order to maintain pay equity in lphi. Through this job evaluation process, lphi ensures employee salaries are equitable relative to each other as well as competitive with the market. All employee classifications and compensation schedules shall be set by the ceo in accordance with the board approved compensation structure. The independent board of directors shall determine compensation and terms and conditions of employment for the ceo using comparability data. The independent board of directors will also ensure that the compensation structure, including salary grades and classifications, is analyzed by ceo, cfo and hr manager on an annual basis. All deliberation and decisions are maintained by compensation committee of the board.

PART X, LINE 2:

Partnership for achieving total health accounts for uncertain tax positions in accordance with financial accounting standards board (fasb) acs 740, income taxes. Fasb acs 740, income taxes, prescribes recognition threshold and measurement process for financial statement recognition of uncertain tax positions taken or expected to be taken in a tax return. The interpretation also provides guidance on recognition, derecognition, classification, interest and penalties, accounting in interim periods, disclosure and transition. Management evaluated path's tax positions for the year ended june 30, 2014, and concluded that the organization had taken no uncertain tax positions that require adjustment to the financial statements to comply with the provisions of this guidance. Path's tax filings are subject to audit by various taxing authorities. Path currently does not have any open audit periods as this is an ititial return.
