Additional Explanations

Amended Form 990

Subsequent to the filng of the original return, the organization determined there were revisions necessary to accurately reflect the activity for the period ending 6/30/14 as follows: interest income of $5 was omitted on the original return. The correct amount should be $793 reflected on form 990, page 1, line 10. Profesional fees in the amount of $10,442 were omitted on the orignal return. The correct amount should be $89,874, form 990, page 10, line 11g, page 10, column a. Insurance expense in the amount of $14,398 was omitted on the original return. The correct total form form 990, page 10, line 23, column a should be $48,782. A cash transfer in the amount of $60,000 satisfied these expenses, as well as created a cash balance at the end of the period in the amount of $35,165, reflected form 990, page 11, line 1, column b. The cash transfer has been included on form 990, schedule n, page 2, part 2, line 1. The correct amount should be 945,900, column c. The organization has determined form 990, page 6, line 6 and 7 did not have the correct response on the original return. Financial Statement Notes

PART X, LINE 2:

Lourdesmont is a non-profit corporation as described in section 501(c)(3) of the internal revenue code and is exempt from federal income taxes. In accordance with the financial accounting standards board guidance on accounting for uncertainty in income taxes, management evaluated lourdesmont's tax positions and concluded that lourdesmont had taken no uncertain tax positions that require adjustment to the financial statements to comply with the provisions of this guidance. With few exceptions, lourdesmont is no longer subject to income tax examinations by the u.s. Federal, state or local tax authorities for years before 2009.

EDUCATIONAL AND CLINICAL STAFF COLLABORATE AS MEMBERS OF A TREATMENT TEAM GIVEN FOCUS BY INDIVIDUALIZED EDUCATIONAL PLANS(IEP) AND INDIVIDUALIZED TREATMENT PLANS(ITP).
