The conflict of interest policy applies to all key employees, as defined by the irs, as well as directors. Additionally, we require all employees to annually certify that they do not have any conflicts that exist were they treated as key employees and to report any such conflicts. An initial determination of any potential related party or excess benefit transaction is made by the general counsel or manager of finance and operations, unless such person is not disinterested. The board is then charged with the review and approval of such transaction and may delegate that responsibility to its audit committee. In all such cases, only disinterested directors may participate.

THE MASSACHUSETTS PFS INITIATIVE WILL IMPROVE THE LIVES OF YOUNG PEOPLE BY PROVIDING INTENSIVE OUTREACH AND SERVICES TO HELP THEM LIVE HEALTHY, PRODUCTIVE LIVES. IT WILL ALSO REDUCE CRIME, PROMOTE SAFER AND STRONGER COMMUNITIES AND SAVE TAXPAYER DOLLARS. AS PROJECT INTERMEDIARY, THIRD SECTOR HAS OVERSEEN IMPLEMENTATION OF THE CONTRACT AND EVALUATION PLAN. AFTER PERFORMING AN EXTENSIVE LANDSCAPE ANALYSIS, THE COUNTY DECIDED TO MOVE TOWARDS A PAY FOR SUCCESS CONTRACT IN TWO ISSUE AREAS WITH THIRD SECTOR SERVING AS CONSULTANT TO THE COUNTY. THE SANTA CLARA COUNTY PAY FOR SUCCESS INITIATIVE FOR THE CHRONICALLY HOMELESS WILL PROVIDE PERMANENT SUPPORTIVE HOUSING FOR APPROXIMATELY 150-200 CHRONICALLY HOMELESS INDIVIDUALS IN THE COUNTY. PROVIDING SUPPORTIVE HOUSING SHOULD IMPROVE THE TARGET POPULATION'S HEALTH AND QUALITY OF LIFE AND ALSO DECREASE ITS USE OF EMERGENCY AND OTHER COSTLY COUNTY SERVICES. THE PROJECT WILL RUN FOR 6 YEARS AND SUCCESS PAYMENTS WILL BE MADE BY THE COUNTY BASED ON THE TOTAL SUCCESSFUL MONTHS OF STABLE TENANCY. AT THE END OF 2014, THE PROJECT WAS PROGRESSING TOWARDS COMPLETION IN 2015. THE SANTA CLARA ACUTE MENTAL HEALTH PFS PROJECT WILL IMPROVE THE COORDINATION OF CARE FOR THE ACUTELY MENTALLY ILL, BY ENSURING PATIENTS ARE IN ACUTE IN-PATIENT SETTINGS FOR THE MINIMUM TIME NECESSARY, AND INSTEAD PROVIDED WITH MORE EFFECTIVE SERVICES IN A LESS-RESTRICTIVE ENVIRONMENT. THE PROJECT WILL SEEK TO CAPTURE CASHABLE SAVINGS FOR THE COUNTY THROUGH A REDUCTION OF BED DAYS PURCHASED FROM CONTRACT PSYCHIATRIC HOSPITALS AND WITHIN ITS IN-PATIENT HOSPITALS. IN DECEMBER OF 2014, THE COUNTY RELEASED A REQUEST FOR PROPOSAL FOR A SERVICE PROVIDER TO PROVIDE ACUTE MENTAL HEALTH SERVICES TO A SUBSET OF SANTA CLARA COUNTY RESIDENTS.
