Through real-world evidence and clinical data collection and analysis, our programs provide novel insights that identify gaps in care and refine best practices to improve the lives of those living with diabetes. T1d exchange supports quality improvement and innovation through its quality improvement collaborative (t1dx-qi), online patient registry, and data-oriented research services. Through a knowledge-sharing and collaboration-focused approach, t1d exchange accelerates real-world impact by providing clinicians, researchers, industry partners, and advocates with the resources and services they need for better decision support and population health management.
The first program of the organization is the development of a type 1 diabetes exchange ("t1d exchange"). The mission of t1d exchange is improve the lives of people touched by type 1 diabetes by facilitating better care and accelerating new therapies on the path to cures. This is intended to be achieved by collaboration between patients with type 1 diabetes, their caregivers, clinicians, and researchers from the field. T1d exchange consists of four components: (1) a clinic network of over 70+ institutions with doctors, and clinicians, who care for more than 150,000 t1d patients, (2) a clinic registry that captures longitudinal data on approximately 26,000+ individuals with both new-onset type 1 diabetes and longstanding type 1 diabetes; (3) a biobank that contains biological fluid and tissue samples from consented participants with type 1 diabetes; and (4) an interactive website for use by participants with type 1 diabetes and their caregivers to share their data. | IRS990/Desc | 0 | THE FIRST PROGRAM OF UNITIO WAS THE CREATION OF THE T1D EXCHANGE. T1D EXCHANGE WAS BORN OUT OF THE NEED TO BUILD A DYNAMIC, MULTI-PURPOSE, REAL-WORLD PATIENT DATA PLATFORM. WHILE A PATIENT REGISTRY WAS CENTRAL TO THE APPROACH, THE VISION WENT FURTHER. TO HELP RESEARCHERS OVERCOME THE MANY OBSTACLES REQUIRED TO ACCELERATE ALL ASPECTS OF DRUG AND DEVICE DEVELOPMENT, THE MODEL OFFERS EASY ACCESS TO AGGREGATED CLINICAL, BIOLOGICAL, PATIENT-REPORTED OUTCOMES AND ELECTRONIC HEALTH RECORD DATA, ALL WHILE FOSTERING COLLABORATION AMONG PATIENTS, PHYSICIANS, RESEARCHERS AND INDUSTRY.THE MODEL INCLUDES THE FOLLOWING COMPONENTS: CLINIC REGISTRY COMPRISING MORE THAN 150 PATIENT PARAMETERS, INCLUDING BOTH PATIENTS REPORTED AND ELECTRONIC HEALTH RECORD DATA FROM OVER 27,000 WELL-CHARACTERIZED TYPE 1 DIABETES PATIENTS RANGING IN AGE, AT THE TIME OF ENROLMENT, FROM UNDER 1 TO 93 YEARS. PARTICIPANTS SPAN A WIDE RANGE OF DEMOGRAPHIC PARAMETERS, SOCIOECONOMIC STATUSES AND CARE REGIMENS. THE REGISTRY HAS BECOME THE MOST CREDIBLE TYPE 1 DIABETES DATA SET IN THE US. ONLINE PATIENT AND CAREGIVER COMMUNITY GLU (WWW.MYGLU.ORG) CONSISTS OF NEARLY 18,000 PATIENTS AND CAREGIVERS WHO FIND PEER-TO-PEER SUPPORT AND EDUCATIONAL MATERIAL, AND CAN ACCESS AND PARTICIPATE IN REAL-TIME RESEARCH. THE GLU COHORT INCLUDES A COLLECTION OF PATIENT-REPORTED OUTCOMES AND IS MADE UP OF APPROXIMATELY 1% (11,500) OF THE US POPULATION OF TYPE 1 DIABETES PATIENTS. GLU IS A KEY RESOURCE FOR REAL-WORLD PATIENT INSIGHT AND RESEARCH. A UNIFIED CLINIC NETWORK OF MORE THAN 230 COLLABORATING CLINICIANS AND COORDINATORS FROM OVER 75 PEDIATRIC AND ADULT SITES IN THE US, WHICH SEES MORE THAN 150 000 UNIQUE TYPE 1 DIABETES PATIENTS. A BIOREPOSITORY WITH BIOSAMPLES FROM OVER 2,000 UNIQUE PARTICIPANTS WITH CLINICAL, DEMOGRAPHIC AND STUDY-DERIVED INFORMATION. A LIVING BIOBANK WITH OVER 2,400 CONSENTED INDIVIDUALS SUPPORTS RESEARCH REQUESTS THAT CAN'T BE MET THROUGH THE EXISTING BIOREPOSITORY COLLECTION.AS A RESOURCE, T1D EXCHANGE COMBINES WELL CHARACTERIZED DATA, BIOSAMPLES, EXPERTISE, AND PATIENT PERSPECTIVES INTO AN EFFECTIVE, SINGLE-ACCESS PROGRAM. T1D EXCHANGE HAS DEMONSTRATED THAT THIS SINGLE ACCESS MODEL IS, IN FACT, DRAMATICALLY IMPROVING THE SPEED AND SUCCESS OF STUDY DEVELOPMENT, PATIENT RECRUITMENT AND STUDY EXECUTION. SINCE 2009, T1D EXCHANGE HAS GONE FROM A VISION TO A WIDELY RECOGNIZED RESOURCE IN THE TYPE 1 DIABETES MEDICAL AND RESEARCH COMMUNITY GLOBALLY. THE EXCHANGE HAS COMPLETED NEARLY 94 SECONDARY OBSERVATIONAL MANUSCRIPTS AND SEVERAL INTERVENTION STUDIES, WITH THIRTEEN STUDIES COMPLETED, SEVEN STUDIES CURRENTLY RUNNING AND TWO IN DEVELOPMENT. TWO EXAMPLES OF THE STUDIES UNDERWAY INCLUDE: INTRANASAL GLUCAGON FOR TREATMENT OF INSULIN INDUCED HYPOGLYCEMIA IN ADULTS WITH TYPE 1 DIABETES: A RANDOMIZED, CROSS-OVER NON-INFERIORITY STUDY; IN ORDER TO UNDERSTAND THE UNMET NEED WITH EXISTING INTRAMUSCULAR GLUCAGON THERAPY, A SURVEY IN 126 PARTICIPANTS FROM THE GLU COMMUNITY WAS COMPLETED OVER 4 DAYS. (RICKELS ET AL, 2015). COGNITIVE DYSFUNCTION AND HYPOGLYCEMIA IN OLDER ADULTS WITH TYPE 1 DIABETES: RESULTS FROM THE T1D EXCHANGE: A STUDY OF OLDER ADULTS (60 YEARS AND OVER) TO SHED LIGHT ON SEVERE HYPOGLYCEMIA. BUILDING FROM INSIGHTS GAINED FROM THE T1D EXCHANGE REGISTRY DATA, RESEARCHERS NOTED THAT INCIDENCE OF SEIZURE AND LOSS OF CONSCIOUSNESS DUE TO SEVERE HYPOGLYCEMIA IN OLDER ADULTS WAS SIGNIFICANTLY MORE FREQUENT THAN PREVIOUSLY UNDERSTOOD. (RIDDLESWORTH ET AL, 2015) A LARGE, GROUND-BREAKING, LONGITUDINAL C-PEPTIDE STUDY, WHICH COLLECTED A FOUNDATIONAL SET OF MORE THAN 1000 UNIQUE SAMPLES, AND GENERATED FINDINGS THAT INDICATED 1 IN EVERY 3 PEOPLE WITH TYPE 1 DIABETES PRODUCES INSULIN YEARS POST DIAGNOSIS, RENDERING SIGNIFICANT CLINICAL AND HEALTH POLICY IMPLICATIONS IN THE US (DAVIS ET AL, 2014).IN ADDITION THESE GROUNDBREAKING RESEARCH STUDIES, THE T1D EXCHANGE CONTINUE TO FOLLOW OUR REGISTRY COHORT AND PROVIDE ONGOING SUPPORT TO OUR 30,000+ LIVES OF PATIENTS, CAREGIVERS AND |
