The organization contracts with an outside firm that provides controllership and financial management services.

Form 990, Part VI, Section B, Line 11B

The 990 is reviewed by the finance committee, treasurer, executive director, and accountant. After this review, the 990 is provided to all board members before filing.

Form 990, Part VI, Section B, Line 12C

At the first board meeting of the year (september), the organization circulates a request form for disclosure each year to board members in order to obtain information regarding conflicts. This form is completed by the respective board members and kept on file. A staff person follows up to obtain any missing forms from board members.

Compensation determined by personnel committee upon annual review with the executive director. Personnel committee had executive director fill out a self-evaluation. The committee met to evaluate completion goals. Personnel committee determined rate of salary increase based on consumer price index. A similar review is performed for key employees.
