2015 bylaws summary of significant changes * removed sections regarding offices and members - already included in the articles of incorporation * replaced the term "national board" with "national governing board of directors" for clarity * modified term limits - no individual can serve three consecutive terms but can serve more than three terms as long as they are not consecutive * the annual meeting for the national governing board and community boards is no longer required to be in the fourth quarter for flexibility * removed limit on the number of community trustees. * the community board may remove a community trustee without needing approval from the national governing board. Reaffirmed that the national governing board may also remove a community trustee.

990 is reviewed in detail by finance committee, including board treasurer before issued to board for review in advance of board meeting. At the board meeting, the treasurer reviews with board and opens up for any questions before motion to approve.

Form 990, Part VI, Section B, Line 12C

All trustees and key staff are required to complete annual disclosures of any potential conflicts. In addition, they are required to disclose new conflicts in a "timely fashion" when they arise. The governance committee reviews disclosed conflicts and assists the board in determining if there is a true conflict of interest. The board (or designee) discusses and decides if the matter is fair and reasonable by majority vote.

Form 990, Part VI, Section B, Line 15

The hr committee conducts salary surveys within the non-profit sector and supplements this data with data from other salary surveys. An extensive organization wide review of salaries and grade levels, lead by hr committee with assistance from hr consultant, was completed in 2014. The ceos compensation is evaluated annually by the hr chair and board chair who utilize external data in determining ceo compensation. In addition, the hr committee and ceo utilize external salary surveys to annualy evaluate the compensation of executive leadership positions.

The consolidated audited financial statements are provided with grant proposals and anyone else who requests them. In addition, a summary of the audited financial statements is provided in the annual report, and both are available electronically through the organization's website. Additional Explanations

Form 990, Part IX

The functional expenses shown on form 990 are for jeremiah program non-profit only. The expenses do not include any expenses from the jeremiah st. Paul limited partnership, which is consolidated with jeremiah program for the consolidated audited financial statements. The partnership files its own tax return. Due to this, the percentage of program expense in relation to total expense is lower on form 990 compared to the consolidated financial statements. On form 990, program expense represents 65% of the total expense, compared to 68% in the consolidated financial statements. In, addition, the percentages are higher with fundraising for program expansion into new communities. Program percentage for the consolidated flagship campuses in twin cities is 79%. Copies of the audited financial statements are available on the organization's website and also available at jeremiah's administrative office on request.

Financial Statement Notes

PART IV, LINE 2B:

The organization collects security deposits from residents to cover potential damage to the units. These deposits will be remitted back to the residents at the end of the lease term net of damages incurred.

PART V, LINE 4:

The endowment funds, established for minneapolis/st. Paul campuses, provide income to support operations. The organization's endowment funds include a board designated endowment, a perpetual trust gifted to the organization, and permanently and temporarily restricted endowment gifts from endowment campaign.

PART X, LINE 2:

The organization is exempt from income taxes under section 501(c)(3) of the internal revenue code and corresponding state tax codes, and therefore the financial statements do not include a provision for income taxes. Contributions to the organization qualify as a charitable tax deduction by the contributor. The organization's 2012-2014 tax years are open for examination by federal and state taxing authorities. The organization files as a tax exempt organization, should that status be challenged in the future, all years since inception would be subject to review by the irs.

PAUL, MN. FOURTEEN JEREMIAH MOTHERS IN MINNEAPOLIS AND ST. PAUL GRADUATED FROM A POST-SECONDARY INSTITUTION. NEARLY ALL (88%) OF GRADUATES WHO MOVED OUT OF JEREMIAH IN 2015 SAID THEY COULD AFFORD SAFE HOUSING AND REPORTED AN AVERAGE WAGE OF $15.78, UP 67% SINCE THEY ENTERED JEREMIAH PROGRAM. EIGHTY-TWO PERCENT (82%) OF THE CHILDREN IN JEREMIAH'S CHILD DEVELOPMENT CENTER, EXCEEDED AGE-APPROPRIATE DEVELOPMENTAL BENCHMARKS. IN AUSTIN, TX, JEREMIAH PROGRAM BROKE GROUND ON A NEW 35-APARTMENT CAMPUS IN NOVEMBER 2015 AND SAW CONTINUED SUCCESSES AMONG THE FIRST COHORT OF FOUR RESIDENT FAMILIES.IN BOSTON, MA, JEREMIAH PROGRAM'S PILOT WITH ENDICOTT COLLEGE ENGAGED 24 FAMILIES WHO SHOWED SIGNIFICANT POSITIVE OUTCOMES, INCLUDING INCREASED GPAS, IMPROVED CONFLICT RESOLUTION SKILLS, AND INCREASED AGE-APPROPRIATE DEVELOPMENTAL SKILLS AMONG THE CHILDREN.IN FARGO, ND-MOORHEAD, MN, JEREMIAH PROGRAM SECURED A LAND DONATION FROM CATHOLIC HEALTH INITIATIVES AND $2.2 MILLION IN FUNDING FROM THE NORTH DAKOTA HOUSING INCENTIVE FUND TOWARD THE BUILDING A NEW CAMPUS FEATURING 20 APARTMENTS AND AN ON-SITE CHILD DEVELOPMENT CENTER. THE ORGANIZATION ALSO PROVIDED EMPOWERMENT TO 21 SINGLE MOTHERS AND COMMUNITY REFERRAL PARTNERS.IN MINNEAPOLIS-ST PAUL, MN, JEREMIAH PROGRAM SERVED 361 PEOPLE, WITH 225 MOTHERS AND CHILDREN IN RESIDENCE. MORE THAN 42% OF RESIDENTS EXCEEDED ACADEMIC BENCHMARKS BY EARNING A GPA OF 3.0 OR HIGHER. A TOTAL OF 14 MOTHERS GRADUATED FROM POST-SECONDARY INSTITUTIONS AND 17 FAMILIES GRADUATED FROM THE PROGRAM, WITH 88% STATING THEY HAD ACCESS TO SAFE AFFORDABLE HOUSING. THIS FLAGSHIP CAMPUS ALSO MADE SIGNIFICANT IMPROVEMENTS TO THE CAREER READINESS PROGRAM AND ACHIEVED CERTIFICATION FROM THE NATIONAL POINTS OF LIGHT HIGH-CAPACITY VOLUNTEER PROGRAM. ACROSS THE UNITED STATES, JEREMIAH PROGRAM PROVIDED TECHNICAL ASSISTANCE TO PARTNERS IN 18 CITIES THAT ARE EVALUATING THE POTENTIAL OF REPLICATING THE PROGRAM IN THEIR COMMUNITIES. JEREMIAH PROGRAM ALSO ADVANCED THE DIALOGUE ON THE POWER OF OUR TWO-GENERATION, ANTI-POVERTY MODEL FOR LOW-INCOME FAMILIES THROUGH NATIONAL PRESENTATIONS TO MULTI-SECTOR LEADERS AND THROUGH INNOVATIVE STUDIES, SUCH AS A NEW JEREMIAH PROGRAM STUDY THAT ASSESSES THE IMPACT OF A FATHER'S INVOLVEMENT ON HIS CHILD'S BEHAVIOR AND READING ACHIEVEMENT. AN ENGAGED COMMUNITY NETWORK OF 300 PHILANTHROPIC ORGANIZATIONS, 1,892 INDIVIDUALS, AND 42 CIVIC GROUP SUPPORTED JEREMIAH FAMILIES AND SERVICES. IN ADDITION, 1,100 VOLUNTEERS CONTRIBUTED NEARLY 17,000 HOURS OF SERVICE VALUED AT $388,926 (SOURCE: INDEPENDENT SECTOR). GIVEN THE GROWING NUMBER OF LOW-INCOME FAMILIES HEADED BY SINGLE WOMEN AND CHILDREN IN POVERTY IN THE U.S., JEREMIAH PROGRAM'S 2016-2018 STRATEGIC PLAN CALLS FOR ACCELERATION, INNOVATION, AND DEMONSTRATION.
