H.o.m.e program:the h.o.m.e. Program provides affordable routine homemaking services (e.g. Dishwashing, laundry, grocery shopping and cleaning), lawn mowing, snow removal, interior and exterior painting, seasonal outdoor work (e.g. Raking, pruning, weeding and mulching), minor home repairs and disability-related projects (e.g. Installing grab bars). H.o.m.e. Services meet seniors' special needs because h.o.m.e. Is a trusted provider and because our services are available on an affordable sliding fee scale. Over 80% of h.o.m.e. Clients are unable to pay the full cost of services. For all of the progam's clients, h.o.m.e. Is a lifesaver that helps them age with dignity and in safety in their own homes, whether a house or an apartment. In 2012, h.o.m.e. Provided 24,554 hours of service to 1,164 seniors.

Activity 3

Senior outreach and caregiver support: licensed social workers provide professional care coordination as well as supportive individual and group counseling for frail seniors and their caregivers. Senior outreach staff help assess strengths and needs, explain options, and link seniors to needed services and resources. The licensed social workers also serve the needs of caregivers of all ages by providing caregiver information and assistance, caregiver coaching, counseling, family meetings, caregiver education and support groups. In 2012, licensed social workers provided supportive counseling to 1,191 seniors and caregivers. Highly trained volunteers and staff offer education and assistance to seniors and their families that equip them to make informed decisions about supplement and part d selections and also provide medical bill sorting, appeals work and information on long-term care. In 2012, volunteers provided education and assistance to 1,419 seniors and caregivers. Through the senior outreach program, seniors are able to remain in their homes for as long as possible and caregivers experience increased levels of confidence in their role, a reduced sense of isolation and improved physical health and emotional well-being.in 2010, senior community services expanded its caregiver services to include carenextion, an innovative "high tech" to "high touch" community-based program that offers everything caregivers need, in one free, easy-to-access program: www.carenextion.org. Caregivers can use the web to organize and coordinate a care team, keep interested people informed about the care receiver's status, find local service providers, get valuable community-specific guidance, or connect by email or phone with a senior community services social worker/family consultant to answer questions and solve problems. The goal of carenextion is to maximize independent living and daily quality of life for seniors through effective caregiver support. Senior partners care (spc) was established in 1973 as minnesota senior federation's first program. The program transitioned to volunteers of america in 2009 when the minnesota senior federation dissolved. Senior community services began administering the program inapril 2012. In 2012, senior partners care helped 1,127 seniors address a critical gap in health care for individuals that have medicare, cannot afford a medicare supplement, and are not eligible for medical assistance. Spc has partnered with most of the major metropolitan area hospitals and hundreds of clinics and providers statewide. These healthcare providers (spc partners) have agreed to consider a waiver of medicare deductibles, coinsurance and co-payments.
