Once a final draft of the form is ready, each member of the board of trustees is e-mailed a copy for review (with schedule b information redacted). All board members are instructed to review the form and provide feedback by a specified deadline.”

Completed questionnaires are returned to the vice president for administration and finance, where they are reviewed for interests that could give rise to conflicts.
