Governance Explanations

Form 990, Part VI, Section B, Line 11

The audited financial statements are approved by the board of directors prior to issuance to required agencies. The form 990 and related reporting is prepared from the audited financials. The board of directors review the 990 prior to filing with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is reviewed annually by both the human resource manager and executive director and any recommendations for changes are presented to the board of directors for approval. Conflict of interest policies apply to all staff members of the agency as well as to the members of the policy council and the board of directors. All supervisors receive a training on the personnel policies and they are required to disclose to the human resource manager any direct circumstances of a conflict of interest or any perceived circumstance that could be construed as a conflict of interest. These disclosures to the human resource manager must be made immediately upon discovery whether the incident is about to occur or if the incident did occur. The human resource manager, in consultation with the executive director, investigates the situation and makes a full report to the executive director who is responsible for reporting to the board any conflicts that have been identified. Training is provided to the governing board members that if there is any conflict of interest or even any appearance of a conflict of interest, that the governing board members must abstain from any discussions of the issue, any final decisions that are made, and any voting on the issue.

Form 990, Part VI, Section B, Line 15

Compensation of the executive director as well as other officers of the organization is reviewed and approved by the governing body/compensation committee. A review of compensation is done each spring and last took place in 2013.

Form 990, Part VI, Section C, Line 19

The organization makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.

IRS990/ProgSrvcAccomActy2Grp/Desc0COMMUNITY SERVICES: MULTIPLE PROGRAMS FALL INTO THE COMMUNITY SERVICES CATEGORY. THE PROMISE NEIGHBORHOOD PROGRAM WORKED TO STRENGTHEN AN IMPOVERISHED COMMUNITY BY COORDINATING SERVICES OF MULTIPLE AGENCIES TOWARDS THE OVERALL GOAL OF IMPROVING SCHOOL READINESS SKILLS AND SCHOOL PERFORMANCE FROM BIRTH UP TO THE COLLEGE YEARS. THE FAMILY FOUNDATIONS PROGRAM IS AN INTENSIVE HOME VISITING PROGRAM THAT WORKS WITH FAMILIES AT OR BELOW THE POVERTY LEVEL TO STRENGTHEN THE FAMILY UNIT AND TO PREPARE YOUNG CHILDREN FOR SCHOOL READINESS. THE JUNEAU COUNTY SAFE AND STABLE FAMILY PROGRAM, THE COLUMBIA COUNTY FAMILY RESOURCE CENTER, AND THE DODGE COUNTY PARENT PLACE WORKED WITH FAMILIES OF ALL INCOME LEVELS WITH AN EMPHASIS ON DEVELOPING POSITIVE PARENTING SKILLS AND PREVENTING SECOND PREGNANCIES FROM OCCURRING AMONG TEEN PARENTS, AND PREVENTING CHILD ABUSE AND NEGLECT FROM OCCURRING. SERVICES INCLUDED HOME VISITS, PARENTING TRAINING, SUPPORT GROUPS FOR PARENTS, AND RESOURCE AND REFERRAL SERVICES FOR FAMILIES. THE COMMUNITY GARDEN PROGRAM ENCOURAGED LOW INCOME FAMILIES TO PARTICIPATE IN LEARNING GARDENING SKILLS TOWARDS AN OVERALL GOAL OF FOOD SUSTAINABILITY. IT IS ESTIMATED THAT MORE THAN 35 FAMILIES HAVE BEEN DIRECTLY SERVED IN ADDITION TO THE COMMUNITY AS A WHOLE SERVICED BY THE PROMISE NEIGHBORHOOD PROGRAM SPECIFICALLY.

IRS990/ProgSrvcAccomActy3Grp/Desc0HOUSING AND HOUSING REHABILITATION: THROUGH THE HOME PROGRAM, RENEWAL IS ABLE TO PROVIDE A SECOND MORTGAGE TO INCOME ELIGIBLE HOMEBUYERS AT A 0% INTEREST RATE AND THAT IS DEFERRED UNTIL THE HOMEBUYER EITHER SELLS THE PROPERTY OR NO LONGER LIVES IN THE HOME AS THEIR PRIMARY RESIDENCE. THIS METHOD BASICALLY BUYS DOWN THE PRIMARY MORTGAGE SO THAT THE PAYMENTS ON THE PRIMARY MORTGAGE ARE MORE AFFORDABLE FOR THE HOMEBUYER. THROUGH A SUB-CONTRACT WITH CENTRAL WISCONSIN COMMUNITY ACTION COUNCIL, CURRENT HOMEOWNERS AND POTENTIAL HOMEBUYERS WHO MEET INCOME ELIGIBILITY GUIDELINES CAN HAVE REHABILITATION COMPLETED TO MAKE THE HOMES THEY ARE CURRENTLY LIVING IN OR THAT THEY ARE LOOKING AT BUYING MEET MINIMUM HOUSING STANDARDS FOR SAFETY AND HABITABILITY. THE HOUSING COST REDUCTION INITIATIVE (HCRI) PROGRAM OFFERS DOWN PAYMENT AND CLOSING COST ASSISTANCE TO POTENTIAL HOMEBUYERS. HOUSING PROGRAMS PROVIDED SERVICES TO 12 INDIVIDUALS AND FAMILIES.
