Form 990, Part VI, Section B, Line 12C

To ensure easter seals southeast wisconsin operates in a manner consistent with charitable purposes and does not engage in activities that could jeopardize its tax-exempt status, periodic reviews shall be conducted. The periodic reviews shall, at a minimum, include whether compensation arrangements and benefits are reasonable based on competent survey information, and the result of arm's length bargaining. Whether partnerships, joint ventures, and arrangements with management conform to the written purposes and do not result in inurement, impermissible private behavior, or in an excess benefit transaction. When conducting the periodic reviews, the organization may, but need not, use outside advisors. If outside experts are used, their use shall not relieve the governing body of its responsibility for ensuring periodic reviews are conducted. Each director, principal, officer and member of a committee with governing body-delegated powers annually sign a statement, which affirms such person has received a copy of the conflicts of interest policy, has read and understands the policy, has agreed to comply with the policy, and understands the organization is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes.

Form 990, Part VI, Section B, Line 15A

The goals of the chief executive officer evaluation process are to clarify expectations of the ceo's role, responsibilities and performance results; assure alignment of organizational priorities and operationalize the strategic plan through actionable work plans; promote objective assessment of the performance of the ceo and changes to ceo's compensation and benefits; provide a climate for personal growth and development of the ceo; provide a model for effective performance management processes for other staff members and assure an evaluation approach consistent with our culture; and provide a method for executing a part of the fiduciary responsibility of directors. The executive evaluation and compensation committee is comprised of directors who, because of their familiarity with the work of the ceo, are in a good position to provide feedback. To ensure a sense of history and continuity over several evaluation periods, it is recommended that this committee be comprised of the current chair, one additional officer, and another current director preferably a standing committee chair. The evaluation of the ceo is intended to provide an objective process of evaluation that promotes healthy dialogue between the ceo and the executive evaluation and compensation committee. As with any process, the effectiveness of the chief executive officer evaluation depends on the execution of its component parts. It is important that the chair act as a champion of the process helping others to understand the benefits and each participant's role in the process. Each participant needs to be fully committed to the goal of making the organization and the ceo as successful as possible. Conscientious participation and confidentiality should be observed throughout the process. Key components of the process include goal setting, which is the establishment of expectations for the ceo's performance in the coming performance period; progress review, which is an interim review of the ceo's progress on performance; and performance evaluation is an evaluation of the ceo's performance against specific objectives for the annual performance period or what has been accomplished. Changes to the ceo's compensation and benefits are then recommended. Goal setting establishes 4-6 key results areas (accountabilities) for the ceo, each with a set of performance objectives designed to establish expectations and determine how successfully the key accountabilities have been executed at the end of the evaluation period. The ceo key result areas should be consistent with the job description and support the easter seals' strategic plan and vision. (the executive evaluation and compensation committee and ceo should agree on the key results areas.) key results areas are unlikely to change significantly from year to year. Examples of key results areas are services and programs, fiscal management, comprehensive development, development of directors, etc. The ceo prepares a draft of performance objectives for each key results area and reviews them with the chair and executive evaluation and compensation committee. Objectives should be based on the short and long-term goals of the organization as outlined in the strategic plan. The key results areas and objectives are finalized and approved. Performance evaluation is a review of results versus objectives. The human resources director provides the chair with easter seals compensation survey, local non-profit surveys, and ceo compensation history for use in reviewing compensation ranges for similarly placed non-profit executives nationally, regionally, and locally. The ceo submits self-appraisal to the executive evaluation and compensation committee including results versus objectives outlined at the beginning of the year. The executive evaluation and compensation committee meets with ceo to review results versus objectives and ask questions. PART V, LINE 4:

The organization's endowment consists of a fund established to benefit the organization for a variety of purposes. The organization's endowment includes funds designated by the governing body of the organization to function as an endowment (quasi-endowment). The organization has set aside certain unrestricted net assets for endowment purposes. As these amounts are not restricted by the donor, but are restricted only by internal policy, the amounts have been classified as unrestricted net assets. The intent is that the amount of unrestricted net assets that are classified as endowment will always be equal to the market value of the funds invested in the endowment investment trust. The governing body may designate additional amounts from time-to-time to be added to the endowment. The governing body may appropriate funds from the endowment to be used for capital investment needs based on a majority vote.

IRS990/Desc0BIRTH TO THREE PROVIDES SERVICES AND SUPPORT FOR INFANTS AND TODDLERS WITH DEVELOPMENTAL DELAYS AND DISABILITIES AND THEIR FAMILIES. SERVICES INCLUDE SPECIAL EDUCATION, OCCUPATIONAL THERAPY, PHYSICAL THERAPY, SPEECH THERAPY AND SERVICE COORDINATION. APPLIED BEHAVIOR ANALYSIS IS A TEACHING MODEL THAT RECOGNIZES A BEHAVIOR THAT REQUIRES ALTERATION THEN IMPLEMENTS A PROCEDURE TO CREATE THAT CHANGE. IN ADDITION, TARGETED BEHAVIORS ARE FREQUENTLY MEASURED AND INTERVENTIONS ARE ALTERED AS NEEDED TO CREATE THE MOST EFFICIENT AND EFFECTIVE OUTCOME. THE OVERALL GOAL OF THIS THERAPY IS TO HELP THE CHILD IMPROVE THEIR COMMUNICATION, DEVELOPMENTAL, SOCIAL, AND INTELLECTUAL SKILLS THROUGH BEHAVIOR SUPPORT. RESPITE SERVICES PROVIDE RECREATIONAL OPPORTUNITIES FOR YOUTH WITH DISABILITIES AGES 7-21 TO GAIN INCREASED INDEPENDENCE AND ENHANCED QUALITY OF LIFE THROUGH ABILITY AND AGE APPROPRIATE ACTIVITIES. MANY OF THESE YOUNG INDIVIDUALS HAVE NOT HAD ACCESS TO FUN AND EDUCATIONAL ACTIVITIES THAT OTHERS ENJOY, AND THEIR FAMILIES HAVE HAD LITTLE OR NO RESPITE TIME FROM THE DEMANDS OF CARE GIVING. THE PROGRAMS ADDRESS THIS ISSUE BY PROVIDING AN ACCEPTING, NURTURING PLACE WHERE THESE YOUNG PEOPLE WITH DISABILITIES CAN SOCIALIZE AND HAVE FUN WITH THREE OPTIONS: SUMMER RESPITE, AFTER SCHOOL RESPITE, AND WEEKEND RESPITE. WE OFFER SUMMER DAY AND RESPITE CAMP PROGRAM OPTIONS IN ASSOCIATION WITH THE MILWAUKEE COUNTY OFFICE FOR PERSONS WITH DISABILITIES AND WAUKESHA PARKS, RECREATION & FORESTRY DEPARTMENT. EACH CAMP PROVIDES QUALITY RECREATIONAL PROGRAMMING WITH OPPORTUNITIES FOR SOCIAL INTERACTION, SELF-EXPRESSION, COMMUNITY OUTINGS AND A VARIETY OF DAILY ACTIVITIES, WHILE OUR CARING STAFF TEND TO CAMPERS' MEDICAL AND PERSONAL CARES. OUR FAMILY SERVICES PROGRAM SUPPORTS FAMILIES WHERE A PARENT, CAREGIVER OR CHILD HAS A DISABILITY. PARENTS AND CAREGIVERS ARE ABLE TO RECEIVE DIRECT SUPPORT WITH PARENTING AND NURTURING SKILLS, CRISIS INTERVENTION AND PREVENTION OF ABUSE AND NEGLECT. CHILDREN ARE ABLE TO IMPROVE THEIR RELATIONSHIP SKILLS THROUGH CREATIVE ACTIVITIES AND MENTORING. 481 INDIVIDUALS WERE SERVED IN THESE PROGRAM DURING THE YEAR.

IRS990/ProgSrvcAccomActy2Grp/Desc0OUR ADULT DAY PROGRAM SUPPORTS ADULTS WITH SEVERE COGNITIVE AND PHYSICAL DISABILITIES AND INTENSE MEDICAL NEEDS WITH WEEKDAY PROGRAMMING AND SPECIALIZED ACTIVITIES. THIS PROGRAM PROVIDES OPPORTUNITIES TO LEARN INDEPENDENCE, MAKE FRIENDS, AND GAIN THE COMMUNITY LIVING EXPERIENCE. THE RECREATION PROGRAM IS THE RESULT OF OUR PARTNERSHIP WITH THE MILWAUKEE COUNTY OFFICE OF PERSONS WITH DISABILITIES. THE PROGRAM PROVIDES SEASONAL RECREATION ACTIVITIES FOR TEENS AND ADULTS, AGES 16 AND OLDER, WITH DISABILITIES AT THE WIL-O-WAY RECREATION CENTERS AND AT OTHER COMMUNITY SITES. THE CARE MANAGEMENT SERVICES PROGRAM IS THE RESULT OF OUR PARTNERSHIP WITH MYCHOICE MYFAMILYCARE. WE ARE RESPONSIBLE FOR MONITORING THE LONG TERM CARE NEEDS AND FEDERAL AND STATE FUNDING SOURCES FOR INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES AND OLDER ADULTS WHO RECEIVE SUPPORT SERVICE IN MILWAUKEE COUNTY. WE ENSURE THAT INDIVIDUALS HAVE THE APPROPRIATE LEVELS OF SUPPORT SERVICES TO LIVE AS SAFELY AND INDEPENDENTLY AS POSSIBLE. OUR CORPORATE GUARDIANSHIP PROGRAMS PROVIDE CRITICAL SUPPORT TO PEOPLE WHO ARE AGING OR HAVE A DISABILITY THAT IMPAIRS THEIR ABILITY TO MAKE INFORMED DECISIONS ABOUT THEIR LIFE AND WELFARE. THESE ARE INDIVIDUALS WITHOUT FAMILY, FRIENDS, OR CAREGIVERS TO ASSIST THEM AND WHO WITHOUT THESE SUPPORTS ARE AT RISK FOR ABUSE AND NEGLECT. WE ARE LICENSED TO ACT AS GUARDIAN OF THE PERSON AND ESTATE TO PROVIDE PERSONALIZED DECISION MAKING REGARDING A PERSON'S QUALITY OF LIFE, HEALTHCARE, FINANCES, AND OTHER LIFE ISSUES. 1,178 INDIVIDUALS WERE SERVED IN THESE PROGRAM DURING THE YEAR.
