IRS990/Description0WHEATON FRANCISCAN SERVICES, INC. (WFSI) is a Catholic, not-for-profit health care and housing organization serving at sites in Wisconsin, Iowa, Illinois, and Colorado. The System includes 15 hospital sites*, three transitional and extended-care facilities, two home health agencies, more than 3,000 physicians, more than 70 clinic sites with 610 employed physicians, and approximately 2,620 units of assisted living and other housing, and the corporate services offices in Wheaton, Illinois, and Glendale, Wisconsin. Started more than 125 years ago and formally incorporated in 1983, WFSI is committed to living out the Mission and Values of its Sponsors, the Wheaton Franciscan Sisters, by providing excellent and compassionate health care and housing services to all, regardless of their ability to pay. WFSI was incorporated in 1983 to preserve and strengthen Judeo-Christian values, provide a framework for lay expertise and involvement, respond to an increasingly complex environment, ensure continuity of Franciscan sponsorship, and assure viability and excellence throughout the organization. As a not-for-profit organization, WFSI reinvests all net income into the communities it serves. Its multiple entities are dedicated to delivering quality health care to patients and residents through highly-trained staff, advanced technology, and superior and compassionate service; caring for each person's body, mind, and spirit with respect and dignity; working to improve the health of the communities; and ensuring health care access for everyone. WFSI is organized into market-based regional holding companies organized in the Southeast, Wisconsin region, as well as other regional holding companies conducting services in Illinois, Iowa, and Colorado. The System's housing and spiritual development services are organized under the holding company name of Franciscan Ministries, Inc. Since 2006, the "doing business as" name for WFSI's health care ministry has been Wheaton Franciscan Healthcare (WFH). 2012 marked the 11th consecutive year Wheaton Franciscan Healthcare in Southeast Wisconsin has been named one of the top 100 most efficient, best performing health care networks in the nation, according to IMS (formerly SDI), the nation's premier integrated health network (IHN) evaluation system. Wheaton Franciscan Healthcare is ranked 32nd and is the top system in Southeast Wisconsin. The top 100 ranking is based on integration in areas such as managed care, information systems, and physician affiliations. Wheaton Franciscan Healthcare scored above the nation's top 100 integrated health networks' average score in the areas of hospital utilization, physician participation, services and access, outpatient utilization, integration, and integrated technology. Other national recognitions include being named one of the best performing 50 systems in Thompson Reuters' 100 Top Hospitals: Health Systems Quality/Efficiency Study in 2009. Wheaton Franciscan Healthcare is a recent recipient of the ECRI Institute's Healthcare Supply Chain Achievement Award. The award honors organizations that demonstrate excellence in overall cost management and in adopting best practice solutions in supply chain processes. Wheaton is one of five health care organizations nationwide to be honored. Wheaton Franciscan Healthcare also received a 2009 VIP Award from McKesson Technology Solutions for its success in using information technology to enhance patient safety across nine hospital sites and more than 100 physician offices. The VIP Awards are presented annually to organizations that demonstrate vision, innovation and performance in the use of healthcare IT. IRS990ScheduleC/Form990ScheduleCPartIV/Explanation0Wheaton Franciscan Healthcare and Franciscan Ministries are part of a controlled group of healthcare and housing organizations that are related through a common parent organization. Certain entities within this controlled group engage in limited lobbying activities that benefit each organization collectively. Wheaton Franciscan Healthcare employs one full time Vice President of Strategic Planning and Government Relations whose duties include a portion of lobbying activities. These lobbying activities approximate 20% of total annual salary. A benefits factor of 25% is added, and the total is allocated amongst the organizations receiving the benefit of these services. The lobbying activities include such things as preservation of Medicare and Medicaid funding; monitoring federal legislation that may impact the organization; participating in and coordinating visits with elected officials; coordinating advocacy activities in conjunction with relevant trade associations; and providing awareness on specific state related legislative issues when the need arises (e.g.: state hospital assessment legislation in Wisconsin). The portion of direct expenses related to annual employee business travel to Washington DC or other locations, in order to lobby for issues important to healthcare providers and patients, have been included if applicable. Finally, any trade association dues containing a percentage portion allocable to lobbying activities, has been added or estimated, as appropriate. (WFSI) maintains the Clara Pfaender Fund, which it established as an endowment fund to encourage WFSI-sponsored ministries and the Wheaton Franciscans to develop programs and services that benefit the poor and disadvantaged. As such, the Clara Pfaender Fund provides funding to seed, expand, or by exception, maintain these efforts in response to specific grant requests all for the purpose of furthering the organization's mission. A fundamental value of the Wheaton Franciscan System is stewardship - providing service to the poor and disadvantaged. The System is committed to respond proactively and tangibly to the needs of the poor, recognizing that often the poor can best be served by empowering individuals and by transforming systems. The Clara Pfaender Fund is designed to help meet this commitment in a direct and positive manner. Grant applications will be evaluated based upon the following criteria: 1. Consistency with the Mission, Vision and Values of the Wheaton Franciscan System. 2. A commitment to serve the poor and disadvantaged, both employees and non-employees, through direct services, empowerment and transformation of values and systems. 3. A commitment to use all grant money to initiate, maintain or expand a ministry which furthers the Mission, Vision and Values of the Wheaton Franciscan System. 4. Preference will be given to the ministerial pathways of health, shelter and social services that are managed by a WFSI organization. 5. Preference will be given to applications that demonstrate the potential to become substantially self-supporting within three years. 6. Preference will be given to ministries that are to be developed within the service areas of the System. 7. Preference will be given to applications that address employee empowerment programs that target development opportunities for elevating employee economic standings where individual or family income falls below the federal poverty guidelines. 8. Grants may be made on an exception basis to ministries beyond the service areas of the System. 9. Grants beyond three years may be given on an exception basis for those clinics that provide 100 percent of their service to the poor. 10. Consideration for grants to community partnerships that meet all of the purposes of Clara Pfaender will be given when a matching grant is made from a WFSI organization. 11. Grants for the same program need to be re-submitted each year. The board of directors in its discretion may waive or modify one or more of these criteria. Grant Application Procedures 1. Proposals shall utilize the approved Grant Application Form, and content shall be limited to the form itself without attachments. Applications shall be submitted by email by the regional holding company president/CEO. If email is not available, the application shall be reviewed and signed by the individual preparing the application and the regional holding company president/CEO prior to being forwarded to the address noted on the application form. 2. If the application is submitted by a Wheaton Franciscan System corporation, it must include the name and telephone number of the person initiating the application, as well as be reviewed by the holding company president/CEO. 3. All applications submitted by Franciscan Ministries, Inc. (FMI), organizations must also be reviewed by the president of FMI 4. If more than one grant request is submitted from a region, the CEO of that region must send a prioritized listing of the grants to the chair of the Clara Pfaender Fund board of directors. 5. Applications should be forwarded to the attention of the assistant to the Clara Pfaender Fund board chairperson at WFSI in Wheaton, Illinois.
