PROVIDED TECHNICAL ASSISTANCE TO FAMILY PATHWAYS IN DEVELOPING A VOLUNTEER BASED PROGRAM TO PROVIDE ASSISTED TRANSPORTATION, RESPITE, CHORE AND HOMEMAKER SERVICES IN MILLE LACS, ISANTI, CHISAGO, KANABEC, AND PINE COUNTIES. TWO OAA PROVIDERS ARE IMPLEMENTING THE REACH COMMUNITY DEMENTIA CAREGIVING INTERVENTION AND TWO ARE IMPLEMENTING THE REST COMPANIONS TRAINING TO TRAIN RESPITE CARE VOLUNTEERS. TECHNICAL ASSISTANCE TO NEWLY FUNDED LIVE WELL GRANTEE, LAKES AND PINES CAP, IN DEVELOPING A HEAVY CHORE PROGRAM. FOSTERING COMMUNITIES FOR A LIFETIME. CO-FACILITATE WITH ESSENTIA HEALTH THE AGING COALITION WHOSE GOAL IS TO DEVELOP AND PROMOTE WELLNESS OPTIONS FOR THE AGING POPULATION IN THE COUNTY. PROVIDING TECHNICAL ASSISTANCE TO BECKER, BRAINERD, CAMBRIDGE, NORTH BRANCH, ONAMIA, PAYNESVILLE, AND SAUK RAPIDS ACT ON ALZHEIMER'S TEAMS AS THEY IMPLEMENT ACTION PLANS. GUIDANCE TO ISANTI, MORRISON, TODD, AND WADENA COUNTIES AS THEY DEVELOP STRATEGIES TO CREATE AWARENESS OF ALZHEIMER'S DISEASE; WITH LITTLE FALLS AND WADENA EMERGING AS DEMENTIA-FRIENDLY COMMUNITIES AND ISANTI AS A DEMENTIA-FRIENDLY COUNTY. PARTNERING WITH THE STEARNS BENTON MEDICAL SOCIETY IN THE CREATION OF A DEMENTIA RESOURCE CENTER TO CONDUCT SCREENING, PROVIDE EDUCATION; CONNECT THEM TO MEDICAL PROFESSIONALS FOR DIAGNOSIS; AND CREATE A PERSON-CENTERED SUPPORT PLAN. |
