Our mission, "Make every moment count for those touched by serious illness and loss," lives in the expert care and support to our patients and their families facing life-limiting illness to live life to its fullest and have the best days possible. In 2014, the organization provided charity care of $1.1 million.

Governance Explanations

Form 990, Part VI, Line 11B Review of form 990 by governing body

There was a merger of three hospice agencies on June 1, 2015. The prior board of directors was dissolved and a new board of directors was formed. Due to the merger, members of the management team of the succeeding organization reviewed and approved the Form 990.

Form 990, Part VI, Line 12C Conflict of interest policy

The conflict of interest policy is reviewed with each new employee and board member during their orientation, along with employees receiving annual training. The policy requires employees and board members to disclose conflicts of interest. A review of vendor payments is completed to identify financial transactions that could result in a conflict of interest.

Form 990, Part VI, Line 15A Process to establish compensation of top management official

An independent third party vendor completes a market analysis which the CEO uses to evaluate the compensation of Executive Directors which is presented to the Board Committee. Human Resources provide the market analysis for the CEO position to the Executive Board Committee and the Committee determines the CEO compensation.

JourneyCare palliative services allows patients to continue curative treatments while our team helps manage difficult symptoms and facilitate discussions about their options, choices, goals for care and quality of life. Our goal is to help you (1) control your pain and symptoms, (2) gain the strength to carry on with daily life, (3) improve your ability to tolerate medical treatments, (4) reduce hospitalizations and emergency room visits, and (5) better understand your choices for medical care.

Grief support is provided through compassionate listening, emotional support and grief education, privately or in group sessions, all in confidence.

The Organization is exempt from income taxes on income from related activities under Section 501(c)(3) of the U.S. Internal Revenue Code and corresponding state tax law. Accordingly, no provision has been made for federal or state income taxes. Additionally, the JourneyCare Foundation has been determined not to be a private foundation under Section 509(a) of the Internal Revenue Code. U.S. GAAP requires that a tax position is recognized as a benefit only if it is "more likely than not" that the tax position would be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized is the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the "more likely than not" test, no tax benefit is recorded. Due to its tax-exempt status, the Organization is not subject to U.S. federal income tax or state income tax. The Organization's Form 990 has not been subject to examination by the Internal Revenue Service or the state of Illinois for the last three years. The Organization does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The Organization recognizes interest and/or penalties related to income tax matters in income tax expense. The Organization did not have any amounts accrued for interest and penalties at December 31, 2014 or 2013.
